Methods: The ultrasound images of 29 cases of mature ovarian teratoma diagnosed by surgery and pathology were summarized and the reasons for misdiagnosis were analyzed.

Results: The misdiagnosis rate of ultrasound diagnosis of mature ovarian teratoma was 7.0%, and the misdiagnosed ultrasound images could be divided into 4 categories: 1. Unilocular or multilocular cystic mass with clear fluid, 14 cases (48.3%); 2. Unilocular cystic mass with thick fluid or irregular strong echo, 2 cases (6.9%); 3. Wall nodule sign, 9 cases (31.0%); 4. Chaotic echo type, 4 cases (13.8%).

Conclusion: When the head section of the wall of mature ovarian teratoma is small or has nerve tissue components, its ultrasound image may not be characteristic, which can easily lead to misdiagnosis, mainly misdiagnosis as ovarian cyst and ovarian cystadenoma. Therefore, during the examination, special attention should be paid to the tiny structure in the cyst to find the typical characteristics of ovarian teratoma, and combined with clinical examination to further improve the accuracy of ultrasound.
