Many people think that health problems with the spleen and stomach are actually nothing and there is no need to pay too much attention to them. The disease will heal on its own and there is no need to worry too much about it. However, when the spleen and stomach are not functioning properly, it will cause problems with the patient's intestinal function and even cause many complications. There will also be depression, abdominal distension and pain, and diarrhea, which will put some burden on the body.

Symptom Analysis

This syndrome is mostly caused by frustrated emotions, anger and depression that damage the liver, liver dysfunction, and spleen and earth; or improper diet, excessive fatigue, damage to the spleen, spleen dysfunction, dampness and wood depression, and liver dysfunction.

When the liver fails to release its qi, the menstrual qi stagnates, causing distension and pain in the chest and flanks. Sighing can lead to the expansion of qi, which can disperse qi stagnation, so the distension and pain can be relieved. When the liver qi is stagnant, the emotions are not smooth, causing depression. Qi stagnation turns into fire, and the liver loses its docile nature, causing impatience and anger. When the liver qi goes against the spleen, the spleen qi is weak and cannot transport and transform food and water, causing abdominal distension after eating less. When qi stagnation and dampness block the flow, there will be rumbling in the intestines, flatulence, loose and uncomfortable stools, or loose and irregular stools. When the liver qi attacks the spleen, qi stagnation occurs, and transportation and transformation are abnormal, so abdominal pain leads to diarrhea. After defecation, the qi can flow smoothly, and the abdominal pain is temporarily relieved after diarrhea. The tongue coating is white and the pulse is stringy or slow, which are symptoms of liver depression and spleen deficiency.

The main basis for diagnosis of this syndrome is chest and flank pain, depression, abdominal distension, loose stools, etc.

Differentiation between liver-stomach disharmony, liver depression and spleen deficiency, and gastrointestinal qi stagnation: the first two are liver qi stagnation, with symptoms such as chest and flank distension and pain, depression or irritability, but liver-stomach disharmony syndrome combined with stomach dysfunction often has symptoms such as epigastric distension and pain, belching, and hiccups; liver depression and spleen deficiency syndrome combined with spleen dysfunction often has symptoms such as loss of appetite, abdominal distension, and loose stools. In the case of gastrointestinal qi stagnation syndrome, the symptoms of liver qi stagnation are not obvious, but only the symptoms of gastrointestinal qi stagnation are seen, with abdominal distension and pain, belching, intestinal rumbling, and flatulence as the main manifestations.

Symptoms of Liver Depression and Spleen Deficiency

To explore the methods and efficacy of TCM in treating irritable bowel syndrome of liver depression and spleen deficiency type. Methods A total of 60 cases of irritable bowel syndrome of liver depression and spleen deficiency type were selected as the treatment group. Tongxie Yaofang combined with Sijunzi Decoction was used as the basic prescription for treatment, and the therapeutic effect was compared with that of the conventional treatment control group. . All cases met the diagnostic criteria for liver depression and spleen deficiency syndrome established in the "Guidelines for Clinical Research on New Chinese Medicines for the Treatment of Irritable Bowel Syndrome":

① Loose stools and lower abdominal distension and pain are related to emotions; ② Emotional anxiety or depression; ③ Poor appetite, fatigue, laziness, and physical weakness. Secondary symptoms: distension and pain in the ribs, or fullness and stuffiness in the stomach; bitter taste in the mouth and dry throat; foreign body sensation in the throat; warm and sour taste; slightly red edges of the tongue tip and slightly yellow tongue coating; or pale tongue, slightly fat tongue or with teeth marks; stringy pulse. Anyone who has any one of the main symptoms ①②③ can be diagnosed with liver depression and spleen deficiency syndrome.
