What is the relationship between prostate enlargement and prostate cancer?

Prostate enlargement and prostate cancer, benign prostatic hyperplasia, also known as benign prostatic hypertrophy, is a common disease in elderly men. The true cause of prostate enlargement is not yet clear, but aging and functional testicles are the basis of the disease. The connective tissue in the prostate of the elderly proliferates and the prostate enlarges, which can compress the urethra and cause dysuria.

Prostate hypertrophy and prostate cancer

Prostate cancer is the most common malignant tumor in men in Europe and America, and the second leading cause of death from malignant tumors in men, second only to lung cancer. The incidence of prostate cancer in my country is relatively low, but has been increasing significantly in recent years. The cause of prostate cancer has not yet been found, but it may be related to genetics, food, environment, sex hormones, etc.

Prostate-specific antigen (PSA) is a protease secreted by prostate epithelial cells, and its content in normal human serum is extremely low. The destruction of normal glandular duct structure in patients with prostate cancer causes an increase in serum PSA content, but PSA may also be slightly elevated in patients with benign prostatic hyperplasia.

PSA exists in the circulation in two forms: free PSA (f-PSA) and bound PSA. f-PSA increases significantly in benign prostatic hyperplasia. Compared with the detection of total PSA (t-PSA) alone, the detection of f-PSA and t-PSA and the calculation of the ratio of the two (f/t) are extremely useful for distinguishing benign prostatic hyperplasia from prostate cancer. In particular, when t-PSA is between 4.0 and 20.0 μg/L, the determination of the f-PSA/t-PSA ratio in serum is more valuable. If the f-PSA/t-PSA ratio is <10%, it indicates prostate cancer, and the f-PSA/t-PSA ratio is >25%, it indicates prostate hyperplasia. Its specificity is 90% and the accuracy is >80%.
