Dental filling includes two main steps: cavity preparation and filling.

The principles of cavity preparation are: ① Remove all diseased tissue, otherwise it will cause secondary caries; ② Protect the dental pulp and healthy tooth tissue; ③ Establish a good retention shape (i.e. a shape that can maintain the filling in the cavity without falling off or loosening) and a resistance shape (i.e. a shape that can withstand chewing force without causing the filling or tooth to break) to prevent the filling from breaking, falling off, or not fitting tightly and causing secondary caries.

Fill and isolate the saliva, disinfect the cavity, prepare the filling material and fill it into the cavity, and carve the shape immediately. Pay attention to removing the part that exceeds the occlusal surface to avoid iatrogenic occlusal trauma. There should be no overhang of fillings on the adjacent surfaces to avoid irritating the gums and causing inflammation.

Selection of filling materials: Filling materials should be non-toxic, wear-resistant, have compressive strength, maintain volume after filling, fit tightly with the cavity, not be corroded or dissolved by saliva, be easy to operate and have beautiful color. However, there is currently no material that fully meets the above-mentioned properties. We can only choose a more appropriate filling material based on the location of the tooth defect, the amount of force applied, and the aesthetic requirements. Generally, silver-mercury alloy is used for back teeth, while resin is used for front teeth, especially those that affect the appearance.

①Silver amalgam .

It is made by mixing and grinding alloy powder containing silver (65%), tin (25%), copper (6%), zinc (2%) and mercury in a weight ratio of 5:8. It is a plastic paste that starts to harden after 15 minutes and completely hardens after 24 hours. Therefore, the filling should be completed within 15 minutes, and the chewing function can only be performed 24 hours after filling. Silver-mercury alloy is silver-gray in color and is not aesthetically pleasing when used to fill front teeth. It is also thermally conductive, so deeper cavities should be padded with adhesive to prevent the conduction of temperature and stimulation of the pulp. In recent years, high-copper alloys have been successfully developed, that is, alloys with increased copper content and reduced silver content. It can improve corrosion resistance, increase strength and reduce volume change.

Amalgam is the most commonly used material for dental fillings because it is strong, durable and has been widely used for more than a century with very satisfactory results.

According to a statement issued by the FDI World Dental Association and the World Health Organization in 1995, the ingredients of dental restoration materials, including silver mercury, will only cause local side effects or allergic reactions in extremely rare cases. There is still no scientific literature indicating that silver mercury is harmful to the human body, so there is no need to worry that silver mercury fillings will affect our health.
