1. Beware of embolism when using hyaluronic acid to augment your chin

Embolism refers to the phenomenon that abnormal substances insoluble in blood appear in normal blood flow, travel to distant places through blood circulation, and block the blood vessel lumen. Substances that block blood vessels are called emboli. Embolism in cosmetic surgery is most common after hyaluronic acid injection, but embolism caused by injection of other soft tissue fillers and autologous fat is also not uncommon.

2. Causes of embolism

1 Direct embolization

During the injection, the needle directly penetrates into the thicker blood vessels, and the hyaluronic acid is directly injected into the blood vessels, causing physical embolism. Because the blood vessels on the face are mostly thin, it is not common in reality.

2 Indirect embolism

During the injection process, the needle tip causes trauma to the blood vessels when it penetrates or is withdrawn, causing blood to flow out slowly. The operator continues to inject hyaluronic acid into the tissue gap outside the blood vessels without noticing. The bleeding and the injection of hyaluronic acid increase the pressure between the tissues. When the external pressure reaches a balance with the systolic pressure of the blood vessels, but exceeds the diastolic pressure of the blood vessels, the blood will flow back, and the mixture of blood and hyaluronic acid in the tissue gap will be pressed back into the blood vessels, forming an embolism.

3. Immediate treatment of embolism

1. If the case is not serious or acute, you can consider using dissolving enzyme to solve the embolism first. The amount of dissolving enzyme injected: should not be too little or too much. The dosage of dissolving enzyme required to completely dissolve a hyaluronic acid is about 200-300 units. Therefore, when injecting the dissolving enzyme dosage, you can refer to how many milliliters of hyaluronic acid were injected into the affected area. Conventional drugs that help restore blood circulation and dilate blood vessels: intravenous drip, oral administration, and topical use.

2. However, when the embolism is acute or severe, it is recommended to first perform multiple wound compression and decompression, and then inject dissolving enzymes into the affected area at multiple points after the compression is almost complete, in order to make up for the residual injection from the embolism compression treatment or to achieve the purpose of prevention. In fact, in clinical practice, the greatest damage of hyaluronic acid embolism is not embolism, but diffusion and metastasis. Once the embolus flows to the small arterial branches, especially the retinal arteries, the risk of blood vessels related to the eyeball and intracranial blood vessels is the greatest. Therefore, preventing its spread as much as possible is the primary goal of early treatment, and distal compression, bloodletting to relieve decompression, and ice compresses to shrink blood vessels are the most critical steps.
