The main reason is the enlargement of hemorrhoid nodules, which separate the mucosa and submucosal layer from the anal layer. During defecation, the internal hemorrhoids prolapse through the anus. In mild cases, the internal hemorrhoids only prolapse outside the anus during defecation. In severe cases, the internal hemorrhoids may prolapse when the abdominal pressure increases, such as by coughing, squatting, exerting force, etc.
