What are the general treatments for nose fractures?

Accidents are everywhere in life, and it is very common to accidentally break your nose due to bumps and collisions. The nose is different from other parts of the body and must be treated with caution, otherwise it is easy to collapse again. From the perspective of clinical treatment effects, external nasal reduction method, intranasal reduction method, open treatment method, etc. have a relatively high success rate and are more popular. Patients can choose one of them for treatment according to their actual condition. During the treatment process, you must pay attention to your diet and daily routine. Do not stay up late, do not eat spicy or irritating foods, and maintain an optimistic attitude, etc. Only in this way can you truly recover.

1. External nasal reduction method. Suitable for nasal bone fractures that are displaced laterally. Under local infiltration anesthesia and nasal mucosal anesthesia, use the thumbs of both hands to press the protruding fracture fragments to reduce them.

2. Intranasal reduction method. Suitable for nasal bone fractures that collapse and displace inward. Under local anesthesia, insert a periosteal separator covered with a rubber tube or wrapped with oil gauze into the nasal cavity, extend the front end of the separator to the fracture site, push the inward fracture fragment forward and outward, and use the thumb and index finger of the other hand to assist in reduction on the outside of the nose. After reduction, iodoform gauze is used to pack the fractured part of the nose to prevent further displacement of the fracture fragments and to help stop bleeding.

3. Open approach

Under local or general anesthesia, first stop the bleeding and then debride the wound. Because the face has a rich blood supply and strong resistance to infection, soft tissue and bone tissue should be preserved as much as possible, and all free bone fragments and foreign objects should be removed. When possible, use catgut to suture the nasal mucosa, pack the nose with vaseline gauze or iodine form gauze, align the fracture, and suture the skin. When the skin defect is not wide enough for suture, the surrounding skin can be freed and tension-relieving sutures can be performed. If the nose wing is defective, the auricular composite tissue transplantation or nasolabial fold flip pedicle flap or "Z" shape plasty can be used.

If there is a sinus fracture, it should be treated according to the principles of sinus fracture treatment. If there is a skull base fracture, neurosurgery should be consulted for cooperation. When there is cerebrospinal fluid rhinorrhea, it is generally not advisable to pack the nostril with gauze. Just put a sterile cotton ball in the anterior nostril and give a large amount of antibiotics systemically to prevent intracranial infection.
