Investigations have shown that concussion is mainly caused by trauma, which triggers temporary, short-term functional disorders in the reticular formation of the brainstem. In the past, it has been believed that concussion is merely a temporary dysfunction of the central nervous system, with no visible organic damage. No lesions were found in gross anatomy or pathological histology. The transient brain function inhibition manifested may be related to factors such as molecular disorders of brain cells, nerve conduction blocks, cerebral blood circulation regulation disorders, damage to interneurons, and cerebrospinal fluid shock waves within the midline ventricles caused by violence. In modern times, according to the electrophysiological research of the nervous system, it is believed that damage to the reticular formation of the brainstem, which affects the function of the ascending activation system, is an important factor causing disorders of consciousness. However, these theories cannot satisfactorily explain all the phenomena of concussion, such as cases of death due to concussion, chronic brain atrophy and even dementia in professional boxers, and reports of mild brain dysfunction in amateur boxers.

2. Pathogenesis

The mechanism of concussion injury is still unclear, and the existing theories cannot fully explain all problems related to concussion. There are many different explanations for the transient disturbance of consciousness after concussion, which may be related to factors such as cerebral blood circulation disorders caused by violence, cerebrospinal fluid impact in the ventricular system, damage to brain interneurons, and abnormal discharges caused by physiological and metabolic disorders of brain cells.
