Blood transfusion is a common treatment method in today's clinical medicine. If the injured or patient is severely ischemic, blood transfusion is needed for treatment. (1) The nurse on duty assists the physician in explaining to the patient and completes the “nine items①” inspection before blood transfusion; (2) Take the blood cross application form and the labeled test tube to the patient's bedside, check the contents of "4 items ②", and then collect 5-6 ml of blood sample; (3) Send the "4 forms"③ and blood samples to the blood bank for verification and double signature by blood bank staff; (4) After the cross-matching is completed, the nurse on duty and the blood bank staff will check the "nine items" together, sign twice, and then take the blood back; (5) When performing a blood transfusion, two medical personnel must bring the medical records to the patient’s bedside to verify the “nine items”④ before blood transfusion can be performed. Adjust the drop frequency to 10-15 drops per minute and explain the precautions for blood transfusion to the patient or family members; (6) Fill out the nursing record sheet (including: transfusion time, blood type, who to check with, etc.) and sign it; (7) After observing for about 10 minutes, adjust the number of drops according to the patient's condition; (8) Observe transfusion reactions and changes in the patient's condition at any time, report any abnormalities to the on-duty physician promptly, and keep records; (9) After the blood transfusion is completed, the nurse on duty will check and confirm that it is correct, then place the blood bag in a double-layer yellow waste bag, fill out the "Nursing Record Sheet" (including: the time of completion of blood transfusion, whether there is a transfusion reaction, whether the patient is uncomfortable, etc.) and sign the "Blood Transfusion Adverse Reaction Report Sheet"; (10) Send the waste bag containing the blood bag and the "Report Form for Adverse Reaction to Blood Transfusion" to the blood bank (the night shift nurse will send the blood bag to the blood bank after get off work the next day) for verification and double signature; (11) After the blood bag has been sealed in the blood bank for one day, if there is no clinical feedback on abnormal blood transfusion, it will be sent to the incinerator for counting and incineration, with double signatures.
