(1) Subacute perforation abscess type: Pathologically, perforation often occurs at the site of the primary cancer, and there is no colon obstruction. Due to the continuous growth of the tumor, the central part of the cancer suffers from nutritional disorders, and necrosis, ulceration, and shedding occur, which communicate with the abdominal cavity. Because the perforation is small, there is no obstruction in the proximal intestinal cavity of the tumor, and the pressure in the intestinal cavity is not high, the amount of colon flowing into the abdominal cavity is small and slow, and the surrounding tissues adhere to form a local encapsulated abscess or inflammatory mass.
Clinically, patients often have no clinical manifestations of acute abdomen in the early stage of perforation. When the abdominal abscess is formed, the patient may experience localized abdominal pain, persistent high fever, elevated blood count, and a lump with obvious tenderness in the abdomen.
(2) Chronic perforation and colon fistula: The invasive growth of colon cancer causes the surrounding tissues and adjacent organs to be affected and adhere to the primary cancer, causing the primary cancer to necrotize and fall off. During perforation, the adjacent affected organs are penetrated, forming various types of colon fistulas.
