Two chemotherapy regimens for Hodgkin's lymphoma

The treatment strategy for Hodgkin's lymphoma is a comprehensive treatment with chemotherapy as the main therapy combined with radiotherapy. The main chemotherapy regimens are MOPP regimen and ABVD regimen.

The use of MOPP regimen for chemotherapy of Hodgkin's lymphoma patients began in 1963. Chemotherapy drugs include nitrogen mustard (M), vincristine (O), procarbazine (P), and prednisone (P). The MOPP regimen chemotherapy cycle is relatively long, requiring at least 6 courses of treatment. The complete remission rate of patients can reach 80%, and the long-term disease-free survival rate can reach 50%. After the relapse of Hodgkin's lymphoma patients who have completely remitted, MOPP regimen chemotherapy is used again, and 59% of patients can still obtain a second complete remission. The adverse reactions of this treatment regimen are the impact on fertility and the occurrence of secondary tumors. According to incomplete statistics, the incidence of the second tumor in patients who have continued chemotherapy for more than 3 months is 3% to 5%, and the infertility rate is 50%.

The ABVD chemotherapy regimen was proposed later than the previous one. The chemotherapy drugs include doxorubicin (A), bleomycin (B), vinblastine (V), and chloramphenicol (D). The remission rate and 5-year disease-free survival rate of this chemotherapy regimen are better than those of the MOPP regimen, and the impact on reproductive function is relatively small, and it does not cause secondary tumors. It has gradually become the first choice chemotherapy regimen for Hodgkin's lymphoma. Since maintenance treatment does not prolong survival, but increases chemotherapy toxicity and suppresses immune function, it is recommended that the ABVD regimen be consolidated for 2 courses after remission, that is, the treatment is terminated.
