Postoperative nursing guidance for laryngeal cancer

Psychological care

Because patients have a certain degree of lack of knowledge about related diseases, and those who have undergone total laryngectomy suddenly lose their ability to speak, and worry that it will bring a series of troubles to their future life, work, and study, they are often pessimistic, negative, and fearful... This leads to unfavorable surgery and even worsens the condition. Therefore, we should talk to patients in a sincere way, explain the necessity of surgery and the methods and precautions for recovering language after surgery, and take advantage of the situation to eliminate their unfavorable psychological factors, so that they can maintain the best psychological state, build confidence in overcoming the disease, and strive for early recovery.

Pathological observation and nursing

Patients under general anesthesia should follow the routine nursing care for general anesthesia (lying flat without pillow and head tilted to one side). After patients under local anesthesia or general anesthesia are fully awake, they should take a semi-recumbent or supine position, and change positions frequently, and pat their backs with hollow hands to prevent pulmonary complications; closely observe changes in vital signs. If there are no contraindications after surgery, the patient should get out of bed and walk around the ward with the help of family members after 1 to 7 days to promote the recovery of various organs of the body; maintain a suitable indoor temperature (temperature around 20°C) and humidity at 60% to 80% to facilitate coughing up sputum; carefully take care of the patient's daily life (getting up and lying down, eating, washing, defecation, etc.) Because the patient cannot pronounce, the patient can be asked to write or make gestures to inform his/her requirements and discomfort); keep the tracheal tube clean and unobstructed, remove secretions in time, and clean and disinfect the inner tube on time, usually once every 6 to 12 hours, and once every hour when there is a lot of secretions, but the inner tube should not be away from the outer tube for too long to prevent the outer tube from being blocked; patients with nasogastric feeding should strictly prevent the nasogastric feeding tube from falling out, and should eat high-calorie, high-protein, and easily digestible liquid food (instruct patients and their families that patients must not eat orally when hungry). Rinse the stomach tube with warm water before and after each meal, and eat once every 3 to 4 hours, 200 to 300 ml each time. Keep the wound clean, change the soaked dressing in time, and if there is a negative pressure suction ball, strictly prevent the plastic tube from falling out, and maintain the pressure of the negative pressure suction ball.

Discharge Instructions

After treatment, the patient's condition stabilizes and he or she can be discharged and go home. We should provide good discharge instructions: advise discharged patients to maintain a happy mood, participate in labor and physical exercise appropriately to enhance physical fitness; minimize activities in public places to avoid cross infection; patients with total laryngectomy should not swim after discharge to prevent suffocation caused by inhalation into the trachea; for those discharged with a tube, the patients and their families should be taught the care methods and key points of the cannula, the methods of cleaning and disinfecting the inner cannula, and the emergency treatment methods for accidents such as tube dislocation before discharge; advise patients to check their necks frequently for lumps, seek medical attention in time if any abnormalities are found, and have regular follow-up examinations.
