Treatment of uterine cancer: Bickenbach (1967) has concluded that surgery alone is better than radiotherapy alone, and its 5-year cure rate is 20% higher for surgery than for radiotherapy. According to the long-term follow-up of 516 cases of endometrial cancer by Zhang Xiyin et al., the survival rate of surgery alone was 72%, while that of those with preoperative radiotherapy plus surgery was 60%.

(I) Ascites or peritoneal washing fluid is used to search for cancer cells; after the peritoneum is cut open, the treatment of uterine cancer is to take out the ascites for the patients with ascites and centrifuge to find cancer cells. For patients without ascites, 200 ml of normal saline is injected into the peritoneal cavity to wash the peritoneal cavity, and the washing fluid is sucked out and centrifuged to find cancer cells. Where cancer cells are found (according to the literature, the rate of stage I cancer is 11.4%, which increases significantly with the increase of tumor grouping, such as 18.1% for grade III). In addition to surgery, other auxiliary treatments should also be added.

(II) Determine myometrial invasion during surgery: For stage I uterine cancer with a uterus smaller than normal, and patients with limited surgery time due to some reasons, uterine adnexectomy can be performed first, and the uterine specimen can be dissected to determine whether there is myometrial invasion. Of course, sometimes the specimen is difficult to judge, and the following subtle changes can be noted under the microscope: ① The glands with myometrial invasion are jagged and irregular in shape, while the glands in the basal layer are round and have no corners; ② There is no endometrial stroma around the glands with cancer invasion, while the glands in the basal layer are often surrounded by endometrial stroma; ③ There is obvious edema around the cancer invasion foci.

If the macroscopic specimen shows that the cancer is located in the lower segment of the uterus, the treatment of uterine cancer should be carried out within the scope of stage II surgery.

(III) Those who are not prepared for lymph node dissection: The treatment of uterine cancer routinely involves exploration of the pelvic and para-aortic lymph nodes. Those with enlargement should at least undergo a biopsy. If the technical conditions permit and the patient allows, lymph node dissection can be performed.

The survival rates after 5, 10, 15, and 20 years of observation were 85.9% to 88.8%, 82.5% to 85.8%, 81.4% to 84.8%, and 77.3% to 81.7%, respectively. This shows the effectiveness of surgical treatment. Surgery can clarify the scope of the lesion and correctly perform clinical staging. The treatment of uterine cancer depends on the correct determination of the scope of surgery. In the past, according to the FIGO staging system in 1982, stage I patients usually underwent extrafascial total hysterectomy plus bilateral adnexectomy.

For stage II, extensive hysterectomy plus bilateral pelvic lymph node dissection is performed. For stage III and IV, surgery is performed first if surgery is possible, and the lesion is removed as much as possible to reduce the size of the tumor. Radiotherapy or progesterone therapy is performed after surgery. Otherwise, progesterone, radiotherapy and/or chemotherapy should be performed first and then surgery is performed when surgery is possible. Other treatments are still required after surgery.
