Ascites is usually caused by cirrhosis and liver cancer. In the late stage of hepatic ascites, the following may occur before death:

1. Gastrointestinal bleeding caused by varicose veins;

2. Pale face, cold limbs, or even shock;

3. Decreased blood pressure due to hepatic ascites;

4. Concurrent hepatic encephalopathy or hepatorenal syndrome.

2. Causes and common diseases

The cause of ascites in cirrhosis is related to the following factors: increased portal vein pressure (over 300 mmH2O), hypoalbuminemia, albumin less than 30 g/L), excessive production of hepatic lymph (normal 1 to 3 L, 7 to 11 L at this time), secondary increase in aldosterone and antidiuretic hormone, and insufficient effective circulating blood volume (increased sympathetic excitability, decreased prostaglandins, atrial peptides and kinins). A large amount of ascites causes the abdomen to swell, appear like a frog belly, and sag.

2. Liver vascular disease

(1) Hepatic veno-occlusive syndrome.

(2) Portal vein thrombosis.

(3) Inferior vena cava obstruction syndrome.

3. Hypoproteinemia

(1) Chylous ascites.

(2) Hypothyroidism.

(3) Meigs syndrome.

3. Treatment principles

1. Treatment of primary disease

The treatment plan for ascites should depend on the underlying disease. For example, tuberculous peritonitis should be treated with anti-tuberculosis treatment; purulent peritonitis caused by perforation of abdominal organs should be treated with surgical treatment; and tumor ascites should be treated with surgical resection, chemotherapy, radiotherapy or interventional treatment according to the condition.

2. Basic treatment

Including bed rest and diet therapy. Ascites indicates the severity of the disease. Regardless of the cause of ascites, basic treatment cannot be ignored.

3. Bed rest

Bed rest is extremely important for the recovery of heart, liver, and kidney functions, and is beneficial to the disappearance of ascites. On the one hand, bed rest can increase hepatic blood flow, reduce the metabolic burden of the liver, and promote the reabsorption of ascites through the lymphatic spaces of the diaphragm; on the other hand, it can increase renal blood flow, improve renal perfusion, and eliminate water and sodium retention.
