The term BNP is a special term in medicine. It mainly refers to brain peptide sodium, which is a substance mainly produced by the ventricles. It is closely related to the normal physiological function of the heart. Therefore, when there are some abnormalities in the values ​​of brain peptide sodium, it is generally a problem with the heart function, so doctors can use brain peptide sodium to infer whether there is a problem with the patient's heart. When left ventricular function is impaired, myocardial dilation causes rapid synthesis and release into the blood, helping to regulate heart function. BNP secreted by cardiomyocytes first exists in the form of a precursor composed of 108 amino acids. When the cardiomyocytes are stimulated, they are broken down by the action of activating enzymes into an inactive linear polypeptide composed of 76 amino acids and an active cyclic polypeptide composed of 32 amino acids, which are released into the blood circulation and are called NT-proBNP and BNP, respectively.

Since the serum/plasma B-BNP level in normal people is extremely low, the increase of B-BNP level has excellent diagnostic value. B-BNP is mainly used to diagnose heart failure, monitor disease progression, evaluate efficacy and prognosis, and is also used to evaluate the recovery of ventricular function in AMI patients after treatment. When the treatment is effective, the BNP level may drop significantly. A continued increase in the BNP level or a continued lack of decrease in the BNP level usually indicates that the patient's heart failure has not been corrected or is further aggravated. In the differential diagnosis of patients with shortness of breath in the emergency room, the B-BNP level can also be used to accurately screen out dyspnea caused by patients other than heart failure. Due to its myocardial specificity, the B-BNP level measurement has a high negative predictive value. BNP is also a very important indicator for the preoperative evaluation of cardiac function in patients undergoing cardiac surgery. Many foreign studies have also shown that B-BNP has important guiding significance for screening high-risk groups. For example, patients with diabetes, hereditary heart disease, hypertension, previous myocardial infarction, and rheumatic heart disease who have undergone valve replacement surgery should undergo regular B-BNP testing to understand their heart function status in a timely manner. Other studies have suggested that elevated B-BNP levels are highly correlated with increased mortality and the risk of re-hospitalization in high-risk patients, and that B-BNP test values ​​are more predictive than left ventricular ejection fraction (LVEF) and VO2 peak. Since B-BNP is currently the only best laboratory test indicator for evaluating heart failure, its detection is rapid, sensitive and specific, and the detection of this indicator can help screen whether an echocardiogram is needed.

Establishing the normal range of plasma BNP concentration in healthy people in various regions has important practical value for the diagnosis of related diseases in patients in various regions; the diagnostic cutoff points of plasma BNP for different cardiovascular diseases are different, and the establishment of diagnostic cutoff points can improve the accuracy of cardiovascular disease diagnosis. The normal value range of plasma BNP in healthy people in Xi'an (microparticle enzyme immunoassay) is 0-38pg/ml; the clinical diagnostic cutoff points of plasma BNP concentration in coronary heart disease group, myocardial infarction group and heart failure group are 4670pg/ml, 7195pg/ml and 11569pg/ml respectively. Each cutoff point has different sensitivity and specificity for the diagnosis of related diseases. There were significant differences in BNP concentrations between different cardiovascular disease groups and the normal group (P<0.01).
