Folliculitis refers to the purulent inflammation caused by Staphylococcus aureus invading the hair follicles. Traditional Chinese medicine has different names for it according to the location and shape of the disease, such as big pearl sore, hairline sore, sheep beard sore, mole cricket boil, mole cricket disease, and eel arch head. The disease is prone to occur on the head, neck, buttocks, perianal area or other parts of the body, and tends to recur. It often occurs in multiple places, is stubborn in nature, and is difficult to cure. This disease is a purulent inflammation caused by bacterial infection of the entire hair follicle. Initially it appears as red papules, which gradually evolve into papular pustules, which are isolated and scattered, and cause mild pain. In adults, it mainly occurs in hairy areas, and in children, it is more common on the head. The rashes can sometimes merge with each other, and small bald spots may remain after healing.

Symptoms

(1) Conscious itching and pain, which is common in young people. There is also sporadic folliculitis on the facial skin.

(2) Papules are millet-sized, with pus forming on the top and not fused. After rupturing, there is a small amount of pus and blood but no pus plugs. No scars are left after healing.

The rash initially occurs at the opening of the hair follicles, with small red follicular papules ranging in size from the tip of a needle to a mung bean that are itchy. A small yellow-white pus head forms on the top of the papule, surrounded by an inflammatory red halo, with hair follicles running through the center. There are many papules, which are scattered and do not fuse with each other. There is mild pain and obvious itching. Generally there are no systemic symptoms. After a few days, the pus head will burst, discharge a small amount of pus, and gradually heal. If the disease recurs and lasts for several weeks, it may turn into chronic folliculitis. It is best used on hairy areas, such as the scalp, perineum, axillae, perianal area and extensor side of the limbs. The rash is likely to occur secondary to diseases such as seborrheic dermatitis, neurodermatitis, and pruritus.

When folliculitis first occurs, it is often scattered, shaped like a grain of rice, and appears as bright red or dark red follicular papules. The center runs through the hair and there is an inflammatory red halo around the edge. They then quickly turn into pustules, but do not merge with each other. When the pustule ruptures or the hair inside it is pulled out, a small amount of pus and plasma may be discharged, but the local inflammation gradually subsides without leaving any scars. Folliculitis often occurs on the head, but other hairy parts of the body may also be affected. Since folliculitis occurs in different parts of the body, its manifestations vary. For example, if folliculitis occurs on the head, hair loss spots ranging from rice grains to fingernail sizes may appear after recovery, and the affected area will no longer grow hair. This is usually called alopecia areata folliculitis. Some folliculitis are clustered, and their deep parts are interconnected and fused into pieces, which is called perforating folliculitis. If folliculitis occurs in the occipital region, it often proliferates and forms papillary nodules due to its special location, which is called papillary folliculitis or occipital nodular folliculitis.

Folliculitis usually occurs in batches at the beginning, and each lesion takes 5 to 7 days to be absorbed and healed. However, new lesions continue to occur, so the course of the disease often lasts for several weeks to several months, or even longer. The symptoms are sometimes mild and sometimes severe, and they recur repeatedly without healing. The degree of itching varies and is sometimes accompanied by slight pain. During examination, the lymph nodes near the affected area are swollen, and fever may occur when the lesion is severe.
