Pit and fissure caries lesions are cone-shaped, with the base toward the dentin and the tip toward the enamel surface. Some caries lesions have no obvious damage to the enamel surface. Caries lesions with this type of clinical characteristics are also called latent caries.

Such deep pits and grooves can easily hide food debris and bacteria, and even the finest toothbrush bristles are only about 0.2 mm in diameter, so it is difficult to clean the pits and grooves by rinsing the mouth and brushing the teeth. The deep pits and fissures are not cleaned, which provides an ecological environment for bacteria to grow and reproduce. The bacteria metabolize and produce acid, which erodes the bottom and walls of the pits and fissures. Over time, it causes enamel demineralization and forms pit and fissure caries.

Since the enamel thickness at the bottom of the pits and fissures is thinner than that on the smooth surface, pit and fissure caries can easily spread to the dentin and even the pulp cavity quickly. Therefore, pit and fissure caries are prone to caries and develop rapidly.

In the early stages of tooth decay, there are no obvious cavities, but mostly black lines on the occlusal surface. Many people think that it is pigmentation, but it is actually caries of the molars.

The enamel thickness at the bottom of the pits and fissures is thinner than that on the smooth surface. Pit and fissure caries can easily and quickly spread to the dentin and even the pulp cavity. Caries can eventually spread to the pulp (dental nerve), causing pulpitis or apical inflammation. If not treated in time, it will lead to tooth loss. The back teeth are responsible for the main chewing function. Once they are lost, the chewing function will be greatly reduced, affecting nutritional intake and growth and development.
