The third is caused by febrile diseases.

Differential diagnosis of febrile illness

Fever refers to a pathological increase in body temperature, which is a systemic response of the human body to pathogenic factors. Generally speaking, oral temperature is above 37.3°C, rectal temperature is above 37.6°C, and axillary temperature is above 37°C [1], and excluding physiological factors, it can be considered as fever [2]. Children, the elderly, women during menstruation, ovulation and pregnancy, diet, strenuous exercise, sudden entry into a high temperature environment, emotional excitement, etc. can all cause a slightly high body temperature.

1. Causes of fever

Infectious fever accounts for the vast majority, including fever caused by various acute and chronic infectious diseases and acute and chronic systemic and local infections.

Non-infectious fever is caused by the following reasons: blood diseases: such as leukemia, malignant reticulocyte disease, etc. Allergic reactions: such as rheumatic fever, drug fever, serum sickness, etc. Malignant tumors: such as malignant lymphoma, cancer, etc. Connective tissue diseases: such as systemic lupus erythematosus, dermatomyositis, polyarteritis nodosa, etc. Physical and chemical injuries: such as heat stroke, fractures after major surgery, large-area burns, sodium pentachlorophenol poisoning, etc. Neurogenic: such as cerebral hemorrhage, autonomic dysfunction. Others: such as hyperthyroidism, severe dehydration or bleeding, sterile abscess, visceral vascular infarction, tissue cell necrosis, etc. In addition, there are febrile illnesses of unknown cause.

2. Classification of fever

In order to facilitate differential diagnosis, fever is divided into acute fever, acute exanthematous fever, acute fever with lung symptoms, periodic fever, long-term fever and chronic low-grade fever based on the severity, degree, course of disease, special fever type and associated main symptoms and signs.

2.1 Acute fever is common. Most of the causes are due to acute infection. Other causes include allergic or hypersensitivity reactions, connective tissue diseases, blood diseases, tissue necrosis and absorption of blood decomposition products, physical and chemical factors, and malignant tumors.

2.1.1 Acute infectious febrile diseases Viral infections: influenza, acute viral upper respiratory tract infection, acute viral hepatitis, epidemic encephalomyelitis, infectious mononucleosis, nephrotic syndrome, hemorrhagic fever; Chlamydia and Mycoplasma infections: ornithosis (psittacosis), Mycoplasma pneumonia; Rickettsial infections: typhus, scrub typhus, Q fever; Bacterial infections: acute focal bacterial infections, sepsis, tuberculosis, typhoid fever, paratyphoid fever, infective endocarditis, lobar pneumonia, Legionnaires' disease, pericarditis; Spirochete infections: leptospirosis, relapsing fever; Parasitic infections: malaria, amoebic hepatitis, acute schistosomiasis, filariasis.

2.1.2 Acute non-infectious febrile diseases: allergic anaphylactic diseases: rheumatic fever, drug fever, serum sickness; connective tissue diseases: acute systemic lupus erythematosus, acute dermatomyositis; tissue necrosis and blood absorption: acute pancreatitis, acute myocardial infarction, acute hemolysis, extensive burns; blood diseases and malignant tumors: acute leukemia, malignant lymphoma, cancer; metabolic disorders: gout attacks, thyroid storm, malignant hyperthermia, malignant syndrome; others: heat stroke, cerebral hemorrhage, Behcet's disease.

2. In short, there are many diseases that cause fever, and the causes of fever are also very complicated. Acute fever is more common. In clinical work, we should identify febrile diseases and make correct diagnoses to facilitate treatment.
