Can I use enema if I have hemorrhoids?

When constipated, many people will use some help to defecate, such as taking medicine or using enema. People with hemorrhoids will worry about whether they can use enema. In fact, it can be used, because the main ingredient of enema is glycerin, which can soften the stool and prevent the recurrence of hemorrhoids. However, you should not rely too much on drugs. Eating more vegetables and fruits and exercising moderately will make you feel much better.

How to use

Suppositories are a commonly used method for treating functional constipation, but most patients report that the effect is poor, and the effect is still not obvious after increasing the dosage of suppositories. The author uses a disposable catheter to inject enema and combines it with abdominal acupoint massage in clinical and home care, with significant results. It is introduced as follows: the patient takes the left side lying position, inserts the disposable catheter into the anus 20~30mm, then connects the enema to the end of the catheter and injects 40~60ml of enema, finally pulls out the catheter, presses the anus with toilet paper with the left hand, and massages the patient's abdominal Xiawan, Liangmen, Zhangmen, Guanyuan, Tianshu, Qichong and other acupoints with the right hand. If the acupoints cannot be selected along the meridians, you can do clockwise circular massage from the ascending colon, transverse colon, and descending colon with appropriate force until the patient feels the urge to defecate.

How to use enema for children:

Because the opening end of the enema is hard and rough, its use in the treatment of constipation in children often causes redness, swelling and pain in the perianal mucosa, and even damages the rectal mucosa, causing pain to the child and making it difficult for the child to cooperate. Therefore, special attention should be paid to the method when using it on children.

Using a 20ml syringe and a disposable scalp needle instead of a suppository to relieve constipation in children is more effective and easy to operate. Children feel comfortable and are easy to cooperate.

The method is: draw all the enema liquid into a 20ml syringe (discard the needle part), then connect a disposable scalp needle (cut off the front end of the scalp needle with the needle part, leaving 2~5cm for spare), apply paraffin oil and insert it for use.

How to improve the effectiveness of enema?

1. Help the patient to lie prone. Those who cannot lie prone can lie on their left side with their hips raised appropriately.

2. Cut off the top of the enema and squeeze out a little glycerin to lubricate the enema into the anus.

3. Hold the bulb of the enema, slowly insert it into the anus until it reaches the neck of the enema, and quickly squeeze the bulb of the enema. At the same time, ask the patient to take a deep breath.

4. After squeezing out all the liquid, hold the gauze in one hand and massage the anus, and quickly pull out the enema shell with the other hand (adults generally need 30~40ml). Instruct the patient to maintain the original position for about ten minutes.

5. For those who complain of abdominal distension and the urge to defecate, they should be guided to continue inhaling and assisted in massaging the anus.

Adverse reactions: prone to dependence.

Side effects: Enemas help defecation by stimulating the intestinal wall to induce a defecation reflex. If used frequently, the more times the rectum is stimulated, the less sensitive it becomes. Once people get used to the drug, they will no longer respond. This is especially true for patients with dry and small stools. Long-term reliance on enemas for defecation will make it more difficult. Enemas cause dryness of the intestinal wall. Frequent use can cause habitual constipation and can also lead to dependence.

So avoid taking medicines and eat some convenient vegetables and fruits, such as watermelon (it is best to eat the seeds together) and bananas.

When to use enema

Enema is used when you feel like having a bowel movement, because it takes 5-10 minutes for the enema to take effect, and then you can have a bowel movement easily. But for more severe constipation, it takes longer to take effect, generally not more than 30 minutes. Therefore, patients should determine the time to use enema according to their specific circumstances.

In addition, enema cannot be used at any time. Some patients with constipation still cannot defecate after using enema in the conventional way.

This is mainly because the defecation reflex is completed by the nerves in the ampulla of the rectum, and the distance between the ampulla of the rectum and the anus can be up to 7 cm. The thin end of the suppository is only 3 cm long. The suppository squeezed into the anus according to the conventional method can only reach the anal canal, which has little stimulating effect on the rectum and the defecation reflex is relatively weak. Therefore, patients with large amount of stool and relatively soft texture may be able to defecate smoothly, but for patients with small food intake, dry and small stool, the effect will be poor or ineffective.

In addition, enema mainly relieves constipation by stimulating the intestinal wall to induce a defecation reflex. If used frequently, the rectum will become less and less sensitive to the stimulation of the enema, and therefore the effect will become worse.
