(ii) Foreign bodies in the laryngopharynx often stay in the epiglottic valley, the root of the tongue, the pyriform sinus, the lateral wall of the laryngopharynx, or behind the arytenoid cartilage. Spray 1% tetracaine on the laryngopharynx and the root of the tongue until surface anesthesia is achieved and the pharyngeal reflex disappears. The surgeon first uses an indirect laryngoscope to carefully see the foreign body, and then selects appropriate foreign body forceps according to the direction of foreign body insertion. The patient then pulls the tongue forward and downward by himself. The surgeon holds the indirect laryngoscope in his left hand and uses the foreign body forceps in his right hand to lower it along the root of the tongue, gradually approaching the foreign body and removing it (). For foreign bodies deep in the pyriformis sinus, if they cannot be removed under indirect laryngoscope, they can be removed under direct laryngoscope.

Precautions

1. Check the foreign bodies in the pharynx patiently and carefully. The foreign bodies may be covered by saliva. Some foreign bodies penetrate into the space between tonsils and posterior pharyngeal columns, and some penetrate deep into the tonsillar crypts. Foreign bodies in the vallecula may be covered by the epiglottis and are not easily exposed. Use nasal forceps or foreign body forceps to push aside the tissue while carefully inspecting.

2. When checking and removing foreign objects, avoid foreign objects from slipping and entering the trachea.

3. If the patient's chief complaint is obvious and no foreign body is found after thorough examination, it can be temporarily treated as a mucosal abrasion and followed up in the outpatient clinic
