The anus is the part that people pay the most attention to, because its position is very critical. It is the outlet for controlling excretion and plays a vital role in people's excretory system. The nerves in the anus are well developed, so people are particularly sensitive to it and can feel obvious stimulation with just the slightest touch. The inside of the anus is a high-incidence area for diseases. Hemorrhoids, proctitis, etc. have a great impact on the anus. Treatment of these diseases must be completed through the anus, and due to the well-developed anal sphincter, anal dilation must be performed during surgical treatment. Prepare the skin before anal dilation and perform an enema with soapy water. Take the KC position or lateral decubitus position, perform routine local disinfection, and apply local anesthesia or sacral anesthesia. After anesthesia, the anorectal ring gradually relaxes, but the fibrous ring does not relax and is generally located in the middle of the fibrous ring. Before finger dilation, there is usually a sense of tightness when two fingers are inserted into the anal canal! When the fiber ring is found, the anus can be dilated again. To reduce the risk of new anal injury, a section of the anal canal can be squeezed between the two index fingers. For anal fissures, if the fissure is posterior, the fissure surface can be squeezed and lifted, and the finger dilation point can be pressed before dilation. When the fibers are expanded, there is a dull paper-tearing sensation. Then, insert your two middle fingers deeper into the anus, using them to press the puborectalis muscle at the back as a fulcrum, and then use your index fingers to expand it, with 3-4 fingers entering the anal canal being the limit.

The basic requirement for anal dilation is to dilate according to the tightness of the anal sphincter. After the operation, the wound may be closed. Generally, you start with one finger first. Each time, an anal dilator with a diameter approximately equal to the index finger is inserted for about three minutes, and then it can be gradually adjusted to make it thicker. The thickest ones are usually only as thick as a thumb. Anal dilation has two benefits for patients after surgery. One is to promote gastrointestinal motility, and the other is to avoid anal stenosis after surgery.

Note:

1. First of all, the anesthesia must be complete. If the anesthesia is not good, the patient will feel great pain during anal dilation, the sphincter will not relax enough, and symptoms of anal canal tear will occur.

2. The anal dilation action should be gentle. Avoid violent and rapid stretching of the anus, as it can easily cause multiple tears of the mucosa and anal canal. Instead, apply force gradually, starting with inserting two fingers into the anus, and then adding fingers one by one. Each additional finger should last about 5 minutes. It is usually appropriate to insert six fingers.

3. When dilating the anus, it is best for men to expand it in the forward and backward directions to avoid contact between the fingers and the ischial tuberosity and affect the dilation of the anus. Because women have wider pelvises, they can expand their anus forward, backward, left and right.

4. Anal dilation at home does not require local anesthesia and can be performed using an anal dilator.
