Heart failure often occurs after acute respiratory tract infection, so it is often accompanied by respiratory failure. Patients experience symptoms of right heart failure such as shortness of breath, palpitations, oliguria, worsening cyanosis, upper abdominal distension and pain, loss of appetite, nausea and even vomiting. Physical examination shows distended jugular veins, increased heart rate, a gallop rhythm audible in the precordial area, or a systolic murmur caused by relative tricuspid regurgitation, which may disappear as the condition improves. Various arrhythmias may occur, especially atrial arrhythmias, hepatomegaly with tenderness, positive liver-neck reverse fluid sign, edema and ascites, and shock may occur in severe cases.
