Lumbar disc displacement

Lumbar disc displacement is still very serious. If it happens, the patient will feel that his waist becomes very painful at the beginning, and he may not be able to straighten his waist at all and walk normally. This will definitely have a huge impact on his normal life. Therefore, when it is discovered in the early stage, you should go to the hospital for a detailed examination and treatment.

⒈ When suffering from lumbar disc herniation, the physiological curve of the lumbar spine decreases or disappears. If combined with lumbar spinal stenosis, kyphosis deformity may occur.

⒉ Scoliosis This is because the lumbar spine bulges toward the affected side, causing tension in the annulus fibrosus on the affected side and partial reduction of the annulus fibrosus, thereby reducing the pressure of the intervertebral disc on the nerve roots. In addition, lumbar scoliosis is also affected by the sacral spine.

⒊ Patients with lumbar disc herniation have more severe gait symptoms, and may walk with a stiff posture, lean forward, or limp.

⒋Tender point. When suffering from lumbar disc herniation, the tenderness point is mainly located beside the spinous process. 1.50 - 3.00 cm from the midline. The deep tissues here are the facet joint capsule, yellow ligament and intervertebral foramen. Lumbar disc herniation (bulging) is caused by tenderness, and the nerve roots are often squeezed between the protrusion and the articular process and yellow ligament, so there is often tenderness here. When tenderness is felt, radiating pain may occur in the lower limbs along the course of the nerve roots, and the pain area is consistent with the plane innervated by the nerve roots. Tenderness may also occur between the spinous processes and spinous process_L, but percussion pain is the main symptom. In addition, clinically, tenderness is often felt in the transverse process of the third lumbar vertebra, as shown in the figure.

⒌ Limited waist movement. When patients with lumbar disc herniation are in the forward flexion position, those with lumbar disc herniation (bulging) will have the nucleus pulposus protrude from the ruptured annulus fibrosus to the rear, aggravating the stimulation and compression on the nerve roots and aggravating the symptoms. However, for those with bulging or protrusion without rupture of the annulus fibrosus, the tension of the posterior longitudinal ligament and the widening of the posterior intervertebral space will cause the nucleus pulposus to move forward, thereby reducing the pressure on the posterior nerve roots and alleviating the symptoms. The pain is relieved when the waist moves toward the healthy side, and the pain is aggravated when the waist moves toward the affected side.

⒍Lower limb muscle atrophy. In case of lumbar disc herniation (bulging), the muscles innervated by the long-term affected nerves may have varying degrees of muscle atrophy. A small number of severe patients may lose the ability to dorsiflex the ankle or thumb.

⒎ Neurological dysfunction in patients with lumbar disc herniation

(l) Sensory nerve disorders: mainly manifested as numbness, pain sensitivity and decreased sensation. Distribution according to the area innervated by the affected nerve roots.

⑵ Motor nerve disorder: Weakened motor strength is a more reliable sign. However, muscle nerves are innervated by multiple nerve roots, so the weakening of muscle strength may not be obvious in some cases.
