The cause of impotence is unclear, because there are many reasons causing impotence. Impotence in young people is mostly caused by psychological conditions. If a man has impotence, it means he cannot have sex and cannot make his wife happy. Therefore, many men suffering from impotence will have a lot of psychological pressure. 1. Central nervous system disorders

Spinal injuries caused by trauma can cause sudden onset of impotence. Complete lesions at the S2 level (upper motor neuron damage) disrupt the connection of the descending motor pathways to the genital area, causing loss of active erectile control, but the erectile and ejaculatory reflexes still exist. However, complete injury to the lumbosacral spine (lower motor neuron injury) results in loss of both the erectile and ejaculatory reflexes. Spinal tumors and degenerative changes in the brain and spine (such as MS and amyotrophic lateral sclerosis) lead to progressive impotence.

(II) Endocrine disorders

Hypofunction of the gland caused by testicular or pituitary dysfunction leads to insufficient secretion of androgen (primary testosterone) and causes impotence. Adrenal cortex and thyroid dysfunction and chronic liver disease can also cause impotence due to their involvement in sex hormone regulation (even if the role is small).

(III) Penile disorders

In patients with Peyronie's disease, the penis is curved, making erections painful and penetration difficult and ultimately unsuccessful. Excessive foreskin hinders erection, which can be improved after circumcision to release the foreskin contraction. Other irritating, infectious, or destructive penile diseases can also cause impotence.

4. Peripheral neuropathy

Systemic diseases, such as chronic renal insufficiency and diabetes mellitus, can lead to progressive impotence if they develop into peripheral neuropathy. This occurs in 50% of men with diabetes. Complications and syndromes of diabetic neuropathy include neurogenic bladder, hypotension, syncope, numbness and other sensory disturbances, muscle weakness, and leg contractures.

5. Psychological Pressure

Itch can arise from a variety of psychological causes, including depression, performance anxiety, recollections of past traumatic sexual experiences, moral or religious conflicts, and uncomfortable emotional or sexual relationships.

(VI) Trauma

Traumatic injuries involving the penis, urethra, prostate, perineum, or pelvis may cause sudden impotence because of structural changes, nerve damage, or disruption of blood supply.

7. Vascular disorders

Various vascular disorders may lead to impotence. These include atherosclerosis affecting the major peripheral vessels, Leriche's syndrome (slowly progressive terminal occlusion of the abdominal aorta), atherosclerosis, thrombosis, or embolism of smaller vessels supplying the penis.
