Dermatofibroma is also known as desmoid tumor. Desmoid tumor is a benign tumor in the dermis caused by focal proliferation of fibroblasts or histiocytes. This disease can occur at any age, but it is more common in young and middle-aged people, and more common in women than in men. 1. Predisposing sites It is more common on the extensor side of the limbs, most often on the lower limbs, above the elbows or on both sides of the trunk and shoulders and back, but can also occur in other parts of the body. 2. Clinical symptoms (1) Skin lesions: They are clinically valuable. They are papules or nodules in the skin. They are raised and hard. The base can be pushed but connected to the epidermis. The surface of the skin is smooth or rough. The color varies. It can be normal skin color or yellow-brown, dark brown or light red. They are more common in middle-aged adults and rarely in children. They are usually round or oval papules or nodules with a diameter of about 1 cm, usually not more than 2 cm, and occasionally 2 cm or larger. The lesions often persist and may disappear on their own after a few years. (2) There are usually no subjective symptoms, but there may be mild pain occasionally. Some patients may develop multiple dermatofibromas, which are associated with lupus erythematosus treated with prednisone or immunosuppressants and HIV infection. (3) Usually single, or 2 to 5, occasionally multiple, nodules with smooth or rough surface and wart-like appearance, usually single, occasionally multiple. 3. Clinical signs It is solid in nature and hard to the touch. It is adhered to the epidermis on the top but not to the deep tissues. It can move freely on the bottom. When pressed laterally, a tiny depression appears in the center of the nodule, which is characteristic. Fitzpatri proposed the name "dimple sign" to indicate its characteristic. The "dimple" sign is positive (pinching the tumor from both sides with the thumb and index finger, the skin above it can be seen that it sinks slightly). The cause is unknown. Some cases may be related to minor local injuries, such as insect bites or blunt injuries. Some people also believe that it is related to viral infection. Ackerman believes that the occurrence of this disease is reactive rather than neoplastic. Yang Meilin et al. (1992) observed through electron microscopy that there are a large number of immature mast cells in close contact with fibroblasts in skin fibromas. Developed and expanded rough endoplasmic reticulum can be seen in fibroblasts, indicating active function. It is believed that the increase in mast cells may be the cause of fibrosis. 1. Since the cause of skin fibroma is not very clear, the prevention methods are as follows: ① Reduce infection as much as possible, avoid exposure to radiation and other harmful substances, especially drugs that suppress immune function; ② Exercise appropriately to strengthen your physical fitness and improve your disease resistance. ③Avoid scratching after mosquito bites. 2. Prevention is mainly aimed at various factors that may cause skin fibroma. It is currently believed that the loss of normal immune surveillance function, the tumorigenic effect of immunosuppressants, the activation of potential viruses and the long-term use of certain physical (such as radiation) and chemical (such as anti-epileptic drugs, adrenocortical hormone) substances may all lead to the proliferation of lymphoreticular tissue and eventually the appearance of skin fibroma. Therefore, pay attention to personal and environmental hygiene, avoid drug abuse, and pay attention to personal protection when working in harmful environments. Western medicine treatment Generally, no treatment is needed, and the lesions will subside within a few years. If necessary, surgical excision is required. Intralesional injection of corticosteroids can also be used for treatment. 1. Generally no treatment is required: If a single lesion is painful and causes distress to the patient, surgical removal can be performed, or cryotherapy can be tried. 2. External treatment of traditional Chinese medicine: such as external application of Veratrum ointment, which can reduce the scope and transfer the poison from deep to shallow; or use ammonium mercuric chloride (white mercury) mixed with rice to make a paste for external application; apply Wanying ointment externally, change the dressing once every 2 to 3 days, so that the tumor will necrotize and fall off, and then use Shengji Powder for local healing. 3. Intradermal injection of corticosteroids: For multiple skin lesions, 1 ml of triamcinolone acetonide (Conacone A) (containing 40 mg of triamcinolone) can be injected into the lesions once every 2 to 3 weeks. If it is difficult to inject more corticosteroids, they can be frozen with liquid nitrogen before injection to make the lesions edematous and soft before hormone injection. Prognosis The lesions often persist or resolve spontaneously after a few years. Some patients may develop multiple cutaneous fibromas. Traditional Chinese Medicine Treatment External treatment methods of traditional Chinese medicine: such as external application of Veratrum ointment, which can reduce the scope and transfer the poison from deep to shallow; or use ammonium mercuric chloride (white mercury) mixed with rice to make a paste for external application; apply Wanying ointment externally, change the dressing once every 2 to 3 days, so that the tumor will necrotize and fall off, and then use Shengji Powder for local healing. 1. Patients with cutaneous fibroma should eat more fresh fruits and vegetables such as cabbage, green vegetables, tomatoes, lotus seeds, Chinese cabbage, water celery, eggplant, soybeans, cucumbers, bean sprouts, apples, bananas, persimmons, mangosteens, oranges, almonds, walnuts, peanuts, pine nuts and other foods. 2. It is advisable to eat diuretic and immunity-enhancing foods. Suitable food: Colostrum It is rich in colostrum protein, which can improve the body's immune function and thus reduce the occurrence of fibroid infection. Houttuynia cordata It has a certain function of increasing immunity, can have a good anti-inflammatory effect, and is beneficial to the disappearance of the inflammation of this disease. Winter melon It has a good diuretic effect and can inhibit the inflammatory response associated with this disease. 1. Eat less health products containing high estrogen and avoid foods containing caffeine such as chocolate and carbonated soda. 2. Avoid starchy pastries and fatty dairy products, hormonal meats and refined sugars, and switch to a low-fat, vegetarian diet, because a high-fat diet not only causes obesity, but also leads to an imbalance in estrogen levels, which is the primary cause of the development. 3. Eat less high-animal fat: Excessive fat intake will accumulate in the breasts, increase the burden on the breasts and aggravate the condition. 4. Eat less greasy food. |
