Classification

Testicular tumors can be divided into germ cell tumors, non-germ cell tumors and testicular secondary tumors. Germ cell tumors are divided into seminoma (35%) and non-seminoma (embryonal carcinoma); teratoma. Choriocarcinoma, yolk sac tumor, etc.) and mixed germ cell tumors. Non-germ cell tumors are divided into interstitial cell tumors, supporting cell tumors, gonadal stromal tumors, and mixed tumors.

Clinical manifestations

The most common symptom is progressive, painless testicular enlargement with a feeling of heaviness, testicular swelling, and induration. Testicular enlargement caused by seminoma tends to maintain testicular contour and consistency, whereas teratoma demonstrates nodular enlargement with an incongruous hardness and tenderness. Approximately 10% of patients experience pain from intratesticular hemorrhage or infarction, and 10% may have metastatic symptoms such as back pain from large retroperitoneal lymph node metastases and nerve root compression. Pulmonary metastases may cause cough and dyspnea, duodenal metastases may cause anorexia, nausea, and vomiting, and bone metastases may cause bone pain. Testicular Leydig cell tumor should be considered in children with testicular masses and symptoms of precocious puberty, or in adults with female breast type and decreased libido.

Physical examination shows swelling of the affected testicle. The testicle is hard and heavy. The light transmission test is negative. The serum concentrations of testicular tumor markers, human chorionic gonadotropin (HCG), and alpha-fetoprotein (AFP) may be increased in patients with seminoma, choriocarcinoma, embryonal carcinoma, and mixed germ cell carcinoma, respectively. B-ultrasound shows that the testicles are more uniform, the echo is enhanced and uneven, and the blood flow signal is strong. Scrotal ultrasound is 100% sensitive to testicular tumors and can show tumors inside and outside the testicle. It can also be used to detect whether there are metastatic tumors in the retroperitoneum, whether there are metastatic lymph nodes in the kidney area, and whether there are metastases in the abdominal organs. It is helpful for tumor staging and observation of therapeutic effects. CT examination can clearly show the relationship between the tumor and the surrounding tissues and determine whether there is metastasis. It is mainly used clinically to observe retroperitoneal lymph node metastasis.

Early Testicular Cancer Symptoms

1. Acute testicular pain

The first early symptom of testicular cancer is sudden pain in the testicles, and the pain is very severe. Doctors explain that the main reason for testicular pain is bleeding from testicular tumor tissue, which causes embolism and ischemic necrosis.

2. Infertility

When a malignant tumor grows on the testicle, it will damage the sperm-producing organ and eventually cause infertility. Therefore, if a man finds himself infertile after a few years of marriage, he should consider testicular cancer.

3. Testicles feel heavy

If a tumor appears in the testicle and is not treated in time, its size will continue to grow, and eventually the testicle will feel heavy, and in the most serious cases, it may even affect normal walking. At this time, if you hold up the testicle with your hand, it will feel like holding up a stone, with a certain sense of weight.

4. Testicular enlargement

Another typical symptom of testicular cancer is swollen testicles, which is mostly discovered when taking a shower. However, there is no pain when the testicles are swollen, only occasional dull pain. However, the feeling of heaviness in the testicles will make people feel very uncomfortable.

5. Acute abdominal pain

The last symptom of testicular cancer is acute abdominal pain. If the patient also has testicular torsion, the acute abdominal pain can be very severe.
