There are several types of fibroids: a. Yellow fibroids: They are more likely to occur in the dermis or subcutaneous layer of the trunk and the proximal upper arm. They often originate from small papules after trauma or itching. The lumps are hard and have unclear edges. They are accompanied by internal bleeding, contain hemosiderin, and are dark brown. Protuberant dermatofibrosarcoma: It is located in the dermis, protrudes from the body surface, has smooth skin, and is similar to a keloid. It is more likely to occur in the trunk. It is low-grade malignant and has a pseudocapsule. It is easy to recur after resection. Multiple recurrences increase the malignancy and can metastasize through the blood. Tumors with enough normal skin and deep adjacent fascia should be removed as soon as possible. c. Band-like fibroids: They are caused by reparative hyperplasia of the abdominal wall muscles after trauma or birth injury. They have no obvious capsule and should be surgically removed. Diagnostic points: 1. They can be seen on all parts of the body, in varying sizes, with smooth surfaces or in the shape of a head, and can be observed freely. 2. Some with pedicles can also be seen, which can grow to several kilograms, are mostly loose and hanging, soft to the touch, and have pigmentation. 3. According to its tissue composition and properties, there are two special types: soft and hard. Soft fibroma, also known as skin tag, is often seen on the face, neck, chest and back, with pedicles, varying in size, soft and elastic. Hard fibroma is more common in women aged 20-40, and is often seen on the abdominal wall. It is hard, painless, non-mobile, and has unclear boundaries with surrounding tissues. It grows slowly, has no capsule and grows in an infiltrative manner. It is easy to recur after removal and can become malignant. 4. Pathological sections can determine the nature of the tumor. Treatment principles: Fibroma should be surgically removed at an early stage, and the surrounding tissues should be appropriately removed. Desmoid tumors should be extensively removed at an early stage, and sent for pathological examination after surgery to rule out malignancy. |
