Chemotherapy methods for endometrial cancer

Endometrial cancer is one of the common malignant tumors in women. The incidence rate has increased significantly in recent years and is showing a trend of becoming younger. The treatment of endometrial cancer is mainly surgical, supplemented by radiotherapy and chemotherapy. In the past, chemotherapy was mainly used to treat advanced and recurrent endometrial cancer. In recent years, the value of chemotherapy in the treatment of endometrial cancer has been increasingly recognized.

1. Chemotherapy for early endometrial cancer. Early endometrial cancer refers to stage I (lesions are confined to the uterine body) and stage II (lesions involve the cervix). The 5-year survival rate can reach 80% to 90%. Such patients are first treated with surgery. Those with the following high-risk factors should receive adjuvant chemotherapy after surgery:

1) Pathological diagnosis histological grade III (poorly differentiated G3);

2) The lesion infiltrates more than half of the uterine muscle wall;

3) The lesion involves more than 50% of the uterine cavity or the lower uterine segment or isthmus is involved;

4) Postoperative pathology confirmed vascular space involvement;

5) Special pathological types, such as uterine serous papillary adenocarcinoma, clear cell carcinoma, etc. Chemotherapy can reduce the rate of distant metastasis in patients with early endometrial cancer and reduce the risk of death by 7%.

2. Chemotherapy for advanced and recurrent endometrial cancer. The prognosis of advanced and recurrent endometrial cancer is very poor, with an average survival time of only 12 months. Chemotherapy is the main treatment method, which can improve the patient's survival rate and can be used as the first choice.
