How to open the pulp of a tooth

Teeth are quite important to humans because they can help people chew food, thereby helping people digest and absorb food. However, many people have certain problems with their teeth and need treatment. Pulp dilation is a necessary step in treating teeth. For different teeth, the method of pulp opening is also different. 1. Maxillary anterior teeth

Anatomical characteristics of the pulp cavity: generally a single root canal, with no obvious boundary between the pulp chamber and the pulp cavity, the root canal is thick, and in the mesiodistal longitudinal section, the distal pulp horn can be seen protruding toward the cutting direction, and in the labiolingual longitudinal section, the protrusion of the pulp chamber near the lingual part can be seen to be enlarged, and the root canal is a round triangle in the cross section of the cervical part of the tooth.

Method of pulp opening: Drill vertically into the tongue above the lingual protuberance, going deeper layer by layer. The drill needle should be slightly extended to the surroundings to avoid breaking. When there is a feeling of missing something, adjust the direction of the needle to be consistent with the long axis of the tooth to enter the pulp cavity, and use a pulling action to remove the top of the pulp chamber to form a triangular cavity with the top facing the root.

2. Mandibular anterior teeth

Anatomical characteristics of the pulp cavity: basically the same as those of the maxillary anterior teeth, except that the tooth size is small and the pulp cavity is small.

Opening follow-up method: When opening the pulp, the needle must be limited to the lingual protuberance and not deviate to the mesiodistal direction. The opening shape is elliptical, and the direction of entering the pulp cavity should be consistent with the long axis of the root canal to avoid mesiodistal lateral penetration.

3. Maxillary premolars

Anatomical characteristics of the medullary cavity: The pulp chamber is cubical, with the buccolingual diameter larger than the mesiodistal diameter. There are two thin and protruding pulp horns extending into the buccolingual apex respectively, dividing it into buccolingual root canals. The root bifurcation is closer to the apical 1/3, and it is difficult to see the pulp chamber floor from the hole. The maxillary first premolars usually have two root canals, the maxillary second premolars may have one root canal, and about 40% have double root canals.

Method of pulp opening: Make an elliptical cavity in the buccolingual direction on the maxillary face, penetrate the buccolingual pulp horns first, do not mistake the two pulp horns just penetrated for the root canal openings, insert the fissure drill and push and grind in the buccolingual direction, connect the buccolingual pulp horns, and then the pulp chamber roof can be uncovered.

4. Mandibular premolars

Anatomical characteristics of the pulp cavity: single root canal, the buccolingual diameter of the pulp chamber and the root canal is large, there is no obvious boundary between the pulp chamber and the root canal, the crown is tilted toward the lingual side, and the top of the pulp cavity is biased toward the buccal side.

Method of pulp opening: Drill at the buccal apex of the maxillofacial region, and be sure not to wear through the mesiodistal wall and the buccal and lingual wall. Always keep the drill needle aligned with the long axis of the tooth.
