The site of perforation caused by intestinal obstruction is often not the site of the tumor, but the proximal end of the obstruction caused by the tumor.
2. Intestinal perforation caused by ulcerative cancer may not occur without intestinal obstruction, but is caused by the continuous growth of the tumor, nutritional disorders in the center of the cancer, tissue necrosis, ulceration, and shedding. This type of intestinal perforation is often clinically manifested by high fever, localized abdominal pain, elevated blood count, and palpable masses in the abdomen, but not necessarily acute abdomen.
3. Infiltrative growth of tumor is also a cause of intestinal perforation. Another cause is the adhesion of tumor to surrounding organs such as bladder, uterus, small intestine, vagina, etc. When the tumor lesions are necrotic and fall off, they can penetrate the affected adjacent organs and form internal fistulas. It is reported that the occurrence of internal fistulas caused by colon cancer accounts for 5% to 28% of all colon cancer perforations.
