What are the manifestations of early gastric cancer under gastroscopy

Early diagnosis of gastric cancer can effectively improve the prognosis of gastric cancer, and gastroscopy is one of the main examination items for early diagnosis of gastric cancer. So, what are the manifestations of early gastric cancer under gastroscopy?

First, regardless of the size of the area, the lesions of early gastric cancer are limited to the mucosa and submucosal layer. Among them, early gastric cancer with a diameter of 5 to 10 mm is called small gastric cancer, and early gastric cancer with a diameter of less than 5 mm is called microgastric cancer.

Secondly, the gross classification scheme for early gastric cancer was first proposed by the Japanese Endoscopic Society in 1962. It has been widely recognized in the industry and has been used to this day.

Type I early gastric cancer: The cancerous site is significantly higher than the surrounding normal mucosa by more than 2 times, or has a polyp-like appearance.

Type II early gastric cancer: The cancerous site is relatively flat, without obvious bulges or depressions. This type of early gastric cancer can be divided into three subtypes according to the degree of convexity and concavity: Type IIa, that is, the cancerous site is slightly higher than the surrounding mucosa, but not more than twice the thickness of the mucosa; Type IIb, the cancerous site is almost the same height as the surrounding mucosa, without bulges or depressions; Type IIc, the cancerous site is slightly depressed compared to the surrounding mucosa, but the degree does not exceed the thickness of the mucosa.

Type III early gastric cancer: The cancerous site is significantly sunken compared to the surrounding mucosa and is accompanied by ulcers, but the cancerous tissue will not exceed the submucosal layer.

In addition, depending on the primary and secondary lesions of early gastric cancer, gastric cancer patients may also present as a mixed type of the above-mentioned types, such as Ⅱc+Ⅲ, Ⅲ+Ⅱc, etc.
