Young people are more at risk of colorectal cancer

Mr. Qiu, 24 years old, was admitted to the hospital for "diarrhea with bloody stools for 3 months and worsening for 1 week"

Medical history

The patient had diarrhea with left lower abdominal pain and bloody stools due to overeating greasy food 3 months ago. Since then, he has had bowel movements 2-3 times a day on average, with loose stools and bloody stools. The abdominal pain worsens after meals. He took norfloxacin on his own, but his condition did not improve. He became noticeably thin and weak in the past week.

Physical examination

The body temperature was 37.8℃. The left supraclavicular lymph node was enlarged, hard, non-tender, and poorly mobile. No obvious abnormality was found in the heart and lungs. A duck egg-sized mass could be felt in the left lower abdomen. Physiological reflexes were present, but pathological reflexes were not elicited.

Testing

Blood routine: WBC 28.9*109/L, RBC 3.02*1012/L, Hb 78G/L, Neutrophil percentage 90.2%

Fiberoptic colonoscopy showed: a neoplastic mass in the sigmoid colon, with a sunken surface and raised edges, and pus and blood. Two pieces of brittle tissue were taken and sent to pathology.

Diagnosis and treatment

Based on the clinical manifestations and pathological results, the patient was diagnosed with poorly differentiated adenocarcinoma of the left large intestine with left supraclavicular lymph node metastasis. A comprehensive treatment plan based on surgery and radiotherapy was recommended.

The patient accepted the treatment plan. Because the patient had relatively extensive metastases and most of the intestines were extensively adhered, a complete radical surgery could not be performed, so a shortcut surgery was performed. The symptoms were alleviated after the operation, but the patient's physical condition was severely exhausted and could not receive corresponding treatment. He was discharged from the hospital after symptomatic treatment. Follow-up one year later showed that the patient had died due to extensive metastasis of cancer throughout the body.

The patient in this case was young, had a high degree of malignancy, was discovered late, had multiple metastases, and eventually died because the best time for treatment was missed.
