Vaginal melanoma is believed to originate from melanocytes in the vaginal mucosa. Melanocytes evolve from embryonic neural crest cells and can be found in the vaginal mucosa of 3% of adult women. During the migration process from the neural crest to the epidermis, melanocytes get lost in the vaginal mucosa. These ectopic melanocytes later become the source of the development of vaginal melanoma.

Clinical symptoms of vaginal melanoma, the most common symptoms are vaginal bleeding (80%), vaginal discharge (7%) or vaginal masses (13%). A small number of patients are asymptomatic, and abnormalities are found in the vagina during physical examination. Some patients with vaginal melanoma have black water-like vaginal discharge, called "black belt". The time from the onset of symptoms to the examination and evaluation varies from 2 days to 4 months, with an average of 2.4 months. When combined with infection, patients with vaginal melanoma may have bloody, foul-smelling, and purulent secretions. In the late stage, they may experience abdominal pain and difficulty urinating and defecating.
