1. T staging of non-small cell lung cancer: The T in the staging of non-small cell lung cancer represents the size of the tumor and the degree of spread in the lungs and adjacent organs.

(1) Tis: The cancer is limited to the lining cells of the airway and has not spread to other lung tissues. This stage of non-small cell lung cancer is often called carcinoma in situ.

(2) T1: The tumor is smaller than 3 cm and has not spread to the visceral pleura. This stage of non-small cell lung cancer does not involve the major bronchi.

(3) T2: The cancer has one or more characteristics: The non-small cell lung cancer tumor is larger than 3 cm; it involves the main bronchus but is more than 2 cm away from the carina; it has spread to the visceral pleura; the cancer partially blocks the airway but does not cause complete lung collapse or pneumonia.

(4) T3: NSCLC at this stage has one or more characteristics: spread to the chest wall, diaphragm, mediastinal pleura or parietal pericardium; involving one main bronchus. The stage of NSCLC is less than 2 cm from the carina but not including the carina; has grown into the airway enough to cause total lung collapse or pneumonia.

(5) T4: The cancer has one or more characteristics: spread to the mediastinum, heart, trachea, esophagus, spine, or carina; two or more independent tumor nodules in the same lobe; presence of malignant pleural effusion.

2. N grading of non-small cell lung cancer: The N stage depends on whether the cancer has invaded certain nearby lymph nodes.

(1) N0: NSCLC is a stage in which the cancer has not spread to the lymph nodes.

(2) N1: Non-small cell lung cancer lymph node spread is limited to the lungs, hilar lymph nodes (located around the place where the bronchi enter the lungs). Non-small cell lung cancer is staged on the same side of the affected lung.

(3) N2: At this stage, the non-small cell lung cancer has spread to the carinal lymph nodes (around the location where the trachea is divided into the left and right bronchi) or the mediastinal lymph nodes (the space behind the sternum and in front of the heart). The involved lymph nodes are limited to the same side of the affected lung.

(4) N3: Cancer has spread to the ipsilateral or contralateral supraclavicular lymph nodes, the stage of non-small cell lung cancer and/or spread to the contralateral hilar or mediastinal lymph nodes of the affected lung.

3. M staging of non-small cell lung cancer: M stands for distant metastasis, which is divided into M0 and M1 according to the presence or absence of metastasis to distant tissues and organs.

(1) M0: Non-small cell lung cancer has not spread to distant sites.

(2) M1: The cancer has spread to one or more distant sites. Distant sites in NSCLC staging include other lung lobes, lymph nodes beyond those mentioned in the N staging, and other organs or tissues, such as the liver, bones, or brain.
