The difference between primary lung tumor and metastatic lung tumor

When a solitary nodule appears in the lung after a history of extrapulmonary tumor, metastatic lung tumor should be considered first. Single nodular lung metastasis should be differentiated from multiple primary lung tumors, i.e., double or multiple primary tumors. Among solitary lung lesions in malignant tumor cases, about 20% to 60% may be primary lung tumors, while true lung metastasis may be only 23% to 46%.

1. Primary lung tumors are sarcomas or malignant melanomas, and most lung masses are metastatic.

2. The primary lung tumor is a well-differentiated squamous cell carcinoma or breast cancer. The lung masses are more likely to be multiple primary cancers.

3. The primary lung tumor is an adenocarcinoma or a poorly differentiated tumor. The possibility of the lung mass being a metastatic tumor is greater or the chance of the primary tumor and the metastatic tumor is equal.

4. If the doubling time of a single pulmonary nodule is <10 days or >1 year, malignant tumors can be ruled out in most cases, but one should still be wary of exceptions.

6. Systemic condition: If cachexia symptoms occur, surgery cannot be considered even if the metastasis is isolated. Surgery cannot solve the problem if there is metastasis to other organs. Surgery is even more impossible for patients with heart, lung, liver, and kidney diseases, and there are many cases of irreversible functional decline. Surgery is not a safe measure.
