⑵Physical examination often shows acute abdomen, with corresponding signs of hydrothorax, upper abdominal tenderness, muscle tension, and even board-like condition. The entry of esophageal and gastric contents into the thoracic and peritoneal cavities can cause chemical thoracotomy and peritonitis, which may present with symptoms of acute suppurative mediastinitis and thoracotomy and peritonitis.

(3) Pain is the most prominent symptom of this disease. It is usually caused by severe vomiting after a full meal or excessive drinking. The patient suddenly experiences unbearable pain in the chest or upper abdomen. It is usually a severe tearing or knife-like pain. The pain worsens with breathing and swallowing, and radiates to the shoulder, back, and behind the sternum on the same side. Severe nausea and vomiting are often interrupted by severe pain. The pain is mostly located in the upper abdomen, but can also be behind the sternum, in the ribs, in the lower chest, and sometimes radiates to the shoulders and back. Patients with cervical esophageal perforation often complain of chest pain, those with mid-esophageal perforation complain of abdominal pain, and those with thoracic and abdominal perforation complain of abdominal and back pain. The pain worsens when swallowing or breathing, and is accompanied by difficulty swallowing, difficulty breathing, and thirst. When the pain is severe, even morphine is difficult to relieve.

(4) Other symptoms: Severe symptoms may include shortness of breath, dyspnea, cyanosis, shock, etc. Dyspnea is associated with pain, pleural dissection, pneumothorax, hydropneumothorax, etc. Due to severe pain, lack of oxygen and blood loss, the patient quickly falls into shock and becomes restless, pale, has cold and clammy skin, a rapid and weak pulse and low blood pressure. If combined with mediastinal thoracic infection, fever may occur, and even sepsis and septic shock may occur.
