Western medicine treatment

The principle of treating gallbladder cancer is comprehensive treatment with surgery as the main method. Its prognosis is mainly determined by the development and staging of the tumor and the choice and thoroughness of the surgical method. Simple cholecystectomy is suitable for early stages, including carcinoma in situ confined to the mucosa. Cholecystectomy is performed without a diagnosis before or during surgery. Pathological sections diagnose it as gallbladder polyps or gallbladder cancer confined to the mucosa. The patient has a very good effect after simple cholecystectomy, with a 5-year cure rate of 70% to 80%.

Standard radical cholecystectomy includes complete cholecystectomy, wedge resection of 2-3 cm of liver tissue from the gallbladder bed, and removal of regional lymph nodes.

Extended radical cholecystectomy is an option for advanced gallbladder cancer. It is a combined surgery that targets different sites of cancer invasion and metastasis, including right hepatectomy, pancreaticoduodenectomy, and portal vein reconstruction. Nakamura reported two cases of advanced gallbladder cancer who underwent right hemihepatectomy, radical cholecystectomy, and hepatoduodenal ligament lymph node dissection. The two cases survived for 12 and 13 years, respectively, without recurrence. This suggests that if the patient's general condition is good, this procedure is expected to improve long-term efficacy.

Palliative surgery, for patients in the late stage with poor general condition and obstructive jaundice, uses Roux-en-Y anastomosis of the common hepatic duct, T-tube drainage, left bile duct jejunostomy or percutaneous transhepatic biliary drainage, aiming to prolong the patient's survival time and improve the quality of life, but the efficacy is not high.

Diet therapy

1. Dietary adjustment for decreased blood count

Chemotherapy for gallbladder cancer can cause bone marrow hypoplasia, especially a decrease in white blood cells. To effectively prevent a decrease in blood count, patients should supplement their high-protein diet during chemotherapy, such as milk, soybeans, lean meat, pig's trotters, sea cucumbers, fish, animal livers, red dates, peanuts, walnuts, black fungus, carrots, red beans, etc. River crabs, black fish, beef, and animal-derived gelatin such as donkey-hide gelatin (ejiao) and pig skin gelatin (roupi gelatin) can also help increase white blood cells.

Traditional Chinese medicine attaches great importance to nourishing the internal organs. Therefore, during chemotherapy, you can also increase the intake of animal bone marrow in appropriate amounts, such as stewing the bone marrow of cows, gooses, and pigs, or making foods with chicken blood, duck blood, goose blood, and pig blood. At the same time, you can also eat more five black foods, such as black sesame, black rice, black beans, black dates and so on. Traditional Chinese medicine believes that "black can enter the kidneys" and that five black foods can nourish the kidneys and fill the marrow, which helps to improve blood counts.

2. Dietary treatment of gastrointestinal toxicity

Chemotherapy for gallbladder cancer can cause oral mucositis, manifested as mucosal congestion, edema, ulcers, pain, etc. At this time, you should keep your mouth clean, brush your teeth after eating, supplement with high-nutrition liquids or a liquid diet, such as lotus seed soup, snow fungus soup, milk, soy milk, crucian carp soup, etc. Avoid overheated, overly acidic and irritating foods when eating. In case of acute inflammation, you can hold ice in your mouth to reduce inflammatory exudation. If ulcers appear, you can add 0.1 gram of crushed vitamin C to 20 ml of honey and hold it in your mouth 2-4 times a day.

Chemotherapy damages the gastrointestinal mucosa and may cause nausea, vomiting, upper abdominal pain, poor appetite, etc. At this time, you can eat appetizers, such as hawthorn, lentils, yam, white radish, mushrooms, etc. At the same time, you should eat small meals frequently to avoid feeling full. Chew food slowly and thoroughly, do not lie flat for 1 hour after a meal, you can spread your legs, do not drink water for 1 hour before chemotherapy, and take 3-5 ml of fresh ginger juice orally if you feel nausea and vomiting while eating.

3. Dietary conditioning and prevention of liver and kidney damage

Some chemotherapy drugs can cause liver damage and elevated transaminases. At this time, you should eat more bitter gourd, mung bean sprouts, tea, shiitake mushrooms, fungus, Hericium erinaceus and other fungal foods, eat more fruits rich in vitamins, such as kiwi, peaches, apples, grapes, etc., and drink more green tea, oolong tea, and honey water.

If the liver function is severely damaged, you can stew crucian carp soup with 20 grams of Schisandra chinensis and 20 grams of wolfberry. Some chemotherapy drugs can also cause kidney damage, such as cisplatin. In clinical practice, when using this type of drug, you should drink more water and eat more fresh vegetables and fruits (alkaline foods). Once renal damage occurs, protein intake should be limited. If edema occurs, eat less salt and eat more animal kidneys, mullet, spinach and red amaranth. You can also eat more foods that are rich in water and have diuretic effects, such as watermelon, cucumber, winter melon and loofah.

Using diet to prevent and treat diseases and maintain health is economical, simple, safe and reliable. These are the characteristics of diet therapy. Diet therapy has a long history. It was recorded as early as "Shennong's Herbal Classic" and "Inner Canon of Medicine", and historians and doctors have also made many discussions on it. Therefore, we should pay attention to diet therapy.
