Urine sediment is the shaped component in urine. It is the sediment formed after the original urine is centrifuged. It is also a combination of the quality and quantity of the shaped components in urine. Urine sediment includes various formed elements such as cells, casts, crystals, bacteria, and sperm.

Examination indicators of urine sediment usually include detection of urinary tract infections and casts.

Urine sediment examination notes:

The patient's urine should be examined under a microscope promptly after being collected. Otherwise, if it is left for too long, hemolysis may occur if the urine is hypotonic; dehydration may occur and the shape of red blood cells may change if the urine is hypertonic; and alkaline urine may cause the collapse of blood cell components and casts, affecting the accuracy of the test.

Normal value

Red blood cells <3/high-power field, white blood cells <5/high-power field. The so-called high-power field of view refers to each field of view when the microscope is magnified 400 times. Tubular casts can be divided into (1) narrow (1-2 red blood cell diameters), (2) medium width (3-4 red blood cell diameters), and (3) wide (5 red blood cell diameters) based on their transverse diameter. Wide casts indicate severe renal parenchyma damage. A few hyaline casts, a few flat epithelial cells, urate, phosphate and oxalate crystals can also be seen in the urine sediment.

To explore the differences between urine analyzer and urine sediment microscopy in detecting white blood cells and red blood cells. Methods Urine sediment was examined microscopically in 85 urine specimens that were all negative by urine analyzer and 281 urine specimens that were occult blood (++)~(++++) by urine analyzer. Results Microscopic examination of 85 urine specimens that were all negative by urine analyzer showed 3-10/HP white blood cells (10.6%) and 1-3/HP red blood cells (4.7%). The detection of 281 urine specimens with occult blood (++)-(++++) by the analyzer revealed that the positive level of occult blood was not synchronized with the red blood cells under microscopic examination, and 1.4% of the occult blood-positive specimens were actually yeast under microscopic examination. Conclusion The quality of urine examination can be improved only by combining urine analyzer with urine sediment microscopy.
