Motor neuron disease often occurs in middle age, has an insidious onset, progresses rapidly, and has no obvious sensory impairment in the early stages.

The clinical manifestations of different types of motor neurone disease include:

⑴ Progressive spinal muscular atrophy: The limbs show symmetrical lower motor neuron paralysis, but at the beginning of the disease, atrophy may be limited to a certain limb or a part of it.

⑵ Primary lateral sclerosis: Symmetrical upper motor neuron paralysis of the limbs, and a few have pseudobulbar palsy.

⑶Amyotrophic lateral sclerosis: more common. ① It starts with atrophy and weakness of small muscles on one side of the hand, gradually spreading to the opposite hand muscles and forearm, upper arm, scapula and other muscles, and there are symptoms of lower motor neuron paralysis such as fasciculations, but tendon reflexes are hyperactive. ②Both lower limbs show typical symptoms of upper motor neuron damage, occasionally manifested in one upper or lower limb.

(4) Auxiliary examinations: ① The electromyogram shows typical denervation changes, occasionally with high-amplitude and long-duration waveforms. ②Muscle biopsy showed typical pathological changes of denervated muscular atrophy.

MRI has a high probability of diagnosing brain or spinal cord parenchymal diseases, such as space-occupying diseases and vascular diseases, or it can be said that it is an anatomical imaging examination.

Motor neuron disease is a group of chronic progressive degenerative diseases that mainly damage the anterior horn of the spinal cord, the motor nuclei of the pontine cranial nerves and the pyramidal tract. Its diagnosis mainly relies on the patient's symptoms and electromyography, and there is no typical manifestation on MRI.
