What to do if the circumcision surgery is not done well

Circumcision is to cut off the long foreskin, that is, to make an incision directly on the back of the penis, and then cut away a circle of excess foreskin horizontally. What should I do if the circumcision surgery is not done well? A foreskin that is too long often affects the sexual life of couples, because the long foreskin will cover the most sensitive part of the male glans, so it cannot get stronger stimulation during sexual intercourse, resulting in an unharmonious sexual life. Neither party can be very happy.

Anesthesia method

Adults use penile base block anesthesia; children use basic anesthesia plus local infiltration anesthesia or general anesthesia.

▍Preoperative preparation

1. Clean the vulva and foreskin cavity. For those with phimosis, a blunt needle or a small plastic tube can be inserted into the foreskin cavity and disinfectant can be injected for flushing.

2. Shave pubic hair one day before or on the day of surgery.

3. Check coagulation function and procaine skin allergy test before operation.

▍Indications

1. Phimosis.

2. After the repair of incarcerated phimosis, the inflammatory edema has subsided and the infection has been controlled.

3. The foreskin is too long and the foreskin opening is small. Although it can be turned over, it is easy to cause phimosis.

4. Patients with excessive foreskin and recurrent balanoposthitis, but the acute infection has been controlled.

5. Benign foreskin tumor.

6. Excessively long foreskin causes repeated or multiple growths of genital warts.

7. In early stage penile cancer, the lesions are small and limited to the foreskin, without deep infiltration and lymph node metastasis.

Contraindications

1. Acute suppurative and gonococcal balanoposthitis and phimosis infection.

2. Patients with hypospadias.

3. People with severe bleeding tendency.

▍Surgical method

1. One-time circumcision of the inner and outer foreskin

(1) Separate adhesions between the foreskin and glans penis: Check whether the foreskin and glans penis are adhered. If so, separate them first.

(2) Traction and fixation of the foreskin: Place the foreskin in a natural position and clamp it with two vascular clamps at the center of the dorsal and ventral sides of the foreskin respectively.

(3) Design the incision and cut the dorsal and ventral foreskin longitudinally.

(4) Circumcision.

(5) Suturing the incision: Suture the inner and outer layers of the foreskin in place.

2. Circular cutting of inner and outer plates

(1) Draw the outer plate incision line and make an incision: With the foreskin tension-free, draw the outer plate incision line parallel to the coronal sulcus at about 0.5 cm distal to the coronal sulcus.

(2) Draw the inner plate incision line and make the incision: Flip the foreskin over the coronal sulcus to expose the coronal sulcus, flatten the inner plate of the foreskin, and draw the inner plate incision line parallel to the coronal sulcus about 0.5 cm proximal to the coronal sulcus.

(3) Foreskin removal: Use a sharp knife to cut the skin along the incision lines drawn on the outer and inner plates of the foreskin, and then make a longitudinal incision on the midline of the dorsal side of the foreskin to connect the transverse incision between the inner and outer plates.
