1. Keep bowel movements smooth

2. Anal canal dilation

It is commonly used in the treatment of acute or chronic anal fissures. Its advantages are simple operation, no need for special equipment, rapid effect, and only daily sitz baths are required after surgery. Method: After local anesthesia, the patient takes a side-lying position, first use the two index fingers to forcefully dilate the anal canal, then gradually insert the two middle fingers, and maintain dilation for 5 minutes. Men should expand forward and backward to avoid contact between fingers and ischial tuberosity, which would affect expansion. Women have a wider pelvis and do not have this problem. After the anal canal is dilated, the spasm of the anal sphincter can be eliminated, so the pain can be relieved immediately after the operation. After dilation, the anal fissure wound expands and opens, drainage is unobstructed, and the superficial wound can heal quickly. However, this method may cause complications such as bleeding, perianal abscess, hemorrhoidal prolapse and short-term fecal incontinence, and its high recurrence rate is its disadvantage.

3. Partial Sitz Bath

This is the most practical and simple method of treating anal fissure.

4. For the treatment of chronic anal fissures that do not heal for a long time and are not responsive to non-surgical treatments, the following surgical treatment methods can be used.

1. Anal fissure resection is to remove the anal fissure and the triangular skin around it. A fusiform or fan-shaped incision is made under local or spinal anesthesia to completely remove the sentinel piles, hypertrophic anal papillae, anal fissures, and vertically cut off part of the internal sphincter if necessary. The advantage of this method is that the lesion is completely removed, the wound surface is wide, the drainage is unobstructed, and granulation tissue is easy to grow from the base. However, its disadvantage is that the wound surface is large and the wound heals slowly.

2. Internal sphincterotomy The internal sphincter has the characteristics of involuntary circular muscles of the digestive tract and is prone to spasm and contraction, which is the main cause of anal fissure pain. Therefore, internal sphincterotomy can be used to cure anal fissure. Partial internal sphincterotomy rarely causes fecal incontinence.
