Vocal cord paralysis is a type of congenital developmental disorder, generally caused by damage to the body's nerves. Vocal cord paralysis can easily affect the brain and cause some brain diseases, which can cause great damage to physical health. There are many ways to treat vocal cord paralysis, which can be treated through vocal cord repair or surgery.

What is the treatment for vocal cord paralysis?

A common cause of vocal cord paralysis is damage to the cricoarytenoid joint during gastroscopy, endotracheal intubation under general anesthesia, or after laryngeal trauma, which can cause cricoarytenoid dislocation and vocal cord paralysis. The cricoarytenoid joint can be manipulated under local anesthesia. After wrapping the throat with gauze, a long curved laryngeal forceps is used to manipulate the arytenoid cartilage to restore cricoarytenoid joint movement and improve vocal cord paralysis. If the bilateral recurrent laryngeal nerves are damaged during thyroid surgery and discovered during or after surgery, recurrent laryngeal nerve transplantation and reconstruction surgery should be performed promptly. If treatment of recurrent laryngeal nerve injury fails, causing vocal cord paralysis and dyspnea, the current treatment plan recommends laser resection of the arytenoid cartilage under general anesthesia supported laryngoscopy to improve breathing. Unilateral or bilateral vocal cord resection is not recommended for the treatment of vocal cord paralysis.

Causes

Depending on the location of nerve damage, it can be divided into central and peripheral, of which peripheral is more common.

(I) Central: The laryngeal motor center of the cerebral cortex on both sides is connected to the nucleus ambiguus on both sides by nerve bundles, so the muscles on each side receive impulses from the cerebral cortex on both sides. Therefore, laryngeal paralysis caused by cortical lesions is extremely rare clinically. Cerebral hemorrhage, basilar artery aneurysm, posterior cranial fossa inflammation, medullary and pons tumors can all cause vocal cord paralysis.

(ii) Peripheral: Any laryngeal paralysis caused by lesions occurring primarily at the recurrent laryngeal nerve or any part of the vagus nerve from the time it leaves the jugular foramen to before it branches out of the recurrent laryngeal nerve is considered peripheral. Skull base fracture, thyroid surgery, various traumas to the neck and larynx, compression by benign or malignant tumors of the larynx, neck or skull base, metastatic tumors of the mediastinum or esophagus, nasopharyngeal carcinoma invading the skull base, tuberculous adhesions at the apex of the lung, pericarditis, peripheral neuritis, etc. can all cause vocal cord paralysis.

Treatment of vocal cord paralysis is directed at its cause. In the case of unilateral incomplete paralysis, there is no obvious obstacle to pronunciation and breathing, and usually no treatment is required. In the case of unilateral complete paralysis, if compensation cannot be achieved after a long time and the patient requires improved pronunciation, Teflon can be injected under the vocal cord mucosa to allow collagen fibers or fat to widen the vocal cords and move them closer to the midline. Bilateral abductor paralysis, if there is difficulty breathing, should be tracheotomy, and then surgical correction.
