Stones are usually painless, so the patient may not know they have a stone unless it causes a blockage or is passed. Bladder or kidney stones can also cause both heavy and microscopic bleeding.

4. Kidney disease: Microscopic urinary bleeding is a common symptom of glomerulonephritis, an inflammation of the kidney's filtration system. Glomerulonephritis may be part of a systemic disease, such as diabetes, or it may occur alone. Glomerulonephritis can be caused by viral or streptococcal infections, vascular disease, and immune problems that affect the small capillaries in the kidneys that filter blood.

5. Cancer: Visible bleeding in the urine may be a sign of advanced kidney, bladder, or prostate cancer. Unfortunately, in the early stages when these cancers are more treatable, patients may have no signs or symptoms.

6. Drugs: Anticancer drugs cyclophosphamide and penicillin can cause urinary tract bleeding. Visible urine can sometimes occur if you take anticoagulant medications such as aspirin and the blood thinner heparin, and your condition can also cause bladder bleeding.

Gross hematuria caused by strenuous exercise is rare and may be related to bladder trauma, dehydration, or red blood cell destruction that occurs during sustained aerobic exercise. Runners are often affected, but anyone can experience noticeable bleeding in the urine after strenuous exercise.
