The buttocks and the back of the thigh is an important part of the human body and it is also where some disease symptoms, such as pain, often occur. People should avoid strenuous exercise in their lives because it can cause some temporary pain symptoms in the human body. So what is the cause of pain under the buttocks and behind the thighs?

There are several reasons for pain in the back of the thigh. One is sciatica. This pain is more likely to occur from the back of the buttocks to the posterior and lateral sides of the thigh and finally to the outside of the calf. This course is called sciatic nerve course. The second is piriformis syndrome, which also compresses the sciatic nerve and causes corresponding symptoms. There are other diseases that can also cause pain, so relevant examinations are needed to confirm the diagnosis and provide timely treatment.

Diagnosis of posterior thigh pain:

The muscles on the back of the thigh include the biceps femoris on the outside and the semitendinosus and semimembranosus on the inside, which are innervated by the sciatic nerve. Therefore, when there is pain in the back of the thigh, the first thing to do is to check whether the femoral flexor muscles are normal? Whether there is a history of strain or other injury to the flexor muscles, in order to determine whether there is any injury to the femoral muscles. The injured parts of the femoral muscles are mostly at the junction of the muscle and the muscle belly, the bifurcation and attachment area of ​​the inner and outer femoral flexor muscles, etc.; muscle belly strain occurs from time to time, and the pain points are mostly in the middle section, and there is often obvious swelling or hematoma, lumps and tenderness on palpation, etc. The pain is aggravated by active, passive contraction or resisted contraction of the muscle.

If the pain is caused by nerves, damage to the sciatic nerve or superior gluteal nerve should be considered first. Injuries to both types of nerves can cause pain in the lower back, buttocks, and posterior thigh, with varying degrees of limitation in leg lifting. However, the pain from injury to the superior gluteal cutaneous nerve is more diffuse, and the location is often vague and uncertain, reaching only the posterior thigh but not the calf. If the sciatic nerve is injured, tenderness may be detected along the course of the sciatic nerve. The pain is not only manifested in the waist, buttocks, and back of the thigh, but also in the entire calf, especially the numbness and pain on the outside of the calf, which is more common. Leg lifting and waist extension are limited, there is pulling pain, tendon reflexes are weakened or disappeared, and in severe cases, muscle atrophy may be seen.

Sciatica can be divided into two types: radicular and trunk. Radicular sciatica is usually caused by lumbar disc herniation or degeneration.

It is more common in L4/5 or L5/S1. Although trunk sciatica can occur along the entire course of the nerve trunk, it is more common in the piriformis muscle, gluteus maximus, medius, and minimus muscles, and gluteal fascia when there is chronic damage. Therefore, the source of sciatica is mainly found in the lower lumbar segment and piriformis muscle. The cause of the superior gluteal cutaneous nerve should be found in the upper lumbar segment.
