1. Overt infection:

The lesions range from smooth, pearly to very irregular edges, with uneven surfaces that overlap each other, and are nipple-like, mushroom-like, cauliflower-like, and cockscomb-like in shape. They are soft in texture, easily bleed when touched, and have a moist surface. They range in size from less than 1mm to 2cm and can fuse into large masses that are prone to erosion and have foul-smelling exudates.

2. Predisposing parts:

In males, it is more common to occur in the glans penis, coronal sulcus, inner side of the foreskin, frenulum of the foreskin, urethral opening and penis; in homosexuals, it is more common to occur in the perianal area and rectum; in females, it is more common in areas other than the genitals, such as the labia majora and minora, vagina, perineum, cervix, perianal area, anorectum, urethra and even groin. At the beginning, there are a few tiny pink papules, which gradually increase in size and number. The surface is often ruptured and eroded due to friction and secondary infection occurs. There are often no symptoms at the beginning, but the affected area may be itched, painful and burning.

3. Milky-smelling discharge

Female patients have increased leucorrhea, even bloody leucorrhea, milky and foul-smelling discharge, and coexistence of gonorrhea, Trichomonas, and fungi. The number or size of lesions will increase during pregnancy, thus affecting delivery, and they often decrease or disappear after delivery. 1% to 25% of patients have urethral damage, which is clearly visible when the urethral opening is turned over or examined with an endoscope, but the lesions do not penetrate more than 3 cm into the urethra. The bladder and its proximal urethra are generally not damaged. The incidence of lesions around the anus in homosexual patients is much higher than that in heterosexual patients. Most genital warts can resolve on their own, but without treatment, the course of the disease can be prolonged and recurrent, and a few may even become malignant.
