What are the causes of painless hematuria

There are many diseases that cause hematuria.

1. Generally, most hematuria is related to genitourinary diseases, including urinary tract infection, stones, prostate disease, tumors, trauma, etc. Most patients with hematuria often have other symptoms. For example, hematuria caused by urinary tract infections such as acute cystitis and urethritis is often accompanied by bladder irritation symptoms such as frequent urination, urgency and pain when urinating; hematuria caused by urinary stones is usually accompanied by acute lumbar colic, etc.

2. When a small number of patients suddenly develop gross hematuria without obvious causes but without pain or other symptoms, it is clinically called "painless gross hematuria". Pan Bainian said that painless macroscopic hematuria is more common in elderly patients and is usually regarded as an important warning sign of urinary system tumors and should be taken particularly seriously. The elderly are a high-risk group for tumors. Common urinary system tumors include bladder cancer, kidney cancer, ureteral cancer, prostate cancer, etc. Among them, bladder cancer is the number one killer, which is manifested by visible hematuria throughout the entire urination process, especially when the hematuria becomes more severe at the end of urination.

Different manifestations of hematuria indicate different sites of lesions

"The time when hematuria appears can reflect the approximate location of the lesion," Pan Boyan said. "Initial hematuria means blood in the urine at the beginning of urination, and then the urine color gradually becomes clear, indicating that the lesions are in the urethra and bladder neck; terminal hematuria means hematuria appears in the last stage of urination, and the urine color is clear in the beginning and middle stages, indicating that the lesions are in the bladder trigone, neck and posterior urethra; full hematuria means blood from the beginning to the end of urination, indicating that the lesion is above the bladder neck. Clinically, there is also urethral hemorrhage, that is, blood flows out of the urethra involuntarily, which is unrelated to urination, and the lesions are mostly at the distal end of the urethral sphincter. Patients with painless macroscopic hematuria must give a detailed medical history, undergo careful examination, and find out the cause as soon as possible."
