Generally, wide local excision is chosen, and the resection margin should be at least 2 to 3 cm away from the edge of the tumor. Patients suspected of malignant transformation should undergo subtotal gastrectomy or total gastrectomy.

Single, pedunculated tumors with a diameter less than 2 cm can be removed by endoscopic electroresection; multiple, pedunculated tumors with a diameter greater than 2 cm or with symptoms such as bleeding or obstruction, or those suspected of malignancy by endoscopic biopsy or cytological examination, should be removed by laparoscopy or laparotomy.

The surgical method can be determined according to the specific conditions of the lesion. For leiomyoma of the gastric body, gastric antrum and gastric fundus with a diameter of less than 5 cm, if the tumor boundaries are clear, there is no necrosis in the tumor, there is no ulcer in the gastric mucosa at the tumor site, and there is no metastasis in the abdominal cavity, local tumor resection can be performed with the cutting margin 1 cm away from the tumor. For leiomyoma located in the pylorus or cardia, conservative local resection or wedge resection together with part of the gastric wall can be performed when the tumor diameter is less than 3 cm.
