What causes spondylitis?

1. The cause of ankylosing spondylitis is related to genetic factors. Family surveys have found that the risk of developing ankylosing spondylitis in the first-generation relatives of ankylosing spondylitis patients is 20 to 40 times higher than that of the general population. A survey of twins found that among identical twins, the probability that the other twin will have the disease is more than 50%. Genetic background is the major determinant of susceptibility to ankylosing spondylitis.

2. The only known susceptibility gene, HLA-B27, has a relative risk of nearly 100%, but it can only explain 10% to 50% of the overall genetic risk of ankylosing spondylitis.

3. Although it has not been confirmed that ankylosing spondylitis is caused by exogenous factors, there is a lot of evidence to support it. The serum IgA antibody level in patients with ankylosing spondylitis is significantly elevated and significantly correlated with the C-reactive protein level. At the same time, the detection rate of Klebsiella pneumoniae in the feces of patients with active ankylosing spondylitis increases, and the serum anti-Klebsiella IgA antibody level and lipopolysaccharide IgA antibody level are increased. There are also literatures indicating that HLA-B27 has the function of stimulating c-fos activity during Salmonella infection. HLA-B27-related reactive arthritis induced by Chlamydia, Salmonella, Shigella, Campylobacter jejuni, Yersinia, etc. has been confirmed.
