1. Subarachnoid hemorrhage is a type of hemorrhagic cerebrovascular disease, but it is different from ordinary cerebral hemorrhage. Ordinary cerebral hemorrhage generally refers to bleeding in the brain parenchyma, while subarachnoid hemorrhage refers to bleeding in the gaps of the brain. The cause of subarachnoid hemorrhage in young patients is mostly intracranial aneurysm and cerebral vascular malformation, while the cause of subarachnoid hemorrhage in elderly patients is mostly aneurysm and cerebral arteriosclerosis. Subarachnoid hemorrhage is the most rapidly progressing and most serious form of hemorrhagic cerebrovascular disease, characterized by severe headache, short-term coma, and the possibility of repeated bleeding.

According to current statistics, the mortality rate of the first subarachnoid hemorrhage in my country is 15%, the mortality rate of the second bleeding is 40%, and the mortality rate of the third bleeding is 70%. At present, when dealing with such patients, hospitals with stronger capabilities usually require the patients to undergo cerebrovascular examinations within a short period of time. If an aneurysm is found, the aneurysm will be promptly filled or clipped and the patient will be relatively safe. If the intracranial aneurysm is not treated, it is equivalent to installing a time bomb in the skull at any time, which may lead to bleeding and endangerment of life. According to what you said, the patient you introduced had multiple bleeding episodes and probably had an intracranial aneurysm. His condition is very critical and he is unlikely to survive. If the bleeding is particularly heavy, he can undergo external ventricular drainage and then slowly remove the aneurysm. There may still be hope, but I am afraid that the patient's condition will change too quickly and I won't give him a chance for surgery.
