1. Open abdominal hyperthermic perfusion chemotherapy (perioperative hyperthermic perfusion chemotherapy)

Most foreign countries use intraoperative indwelling peritoneal catheters and continuous treatment after surgery (using extracorporeal heating). This method can be divided into closed and open. The former performs extracorporeal heating perfusion after closing the abdominal cavity; the latter uses a ring tractor, that is, the abdominal wall is lifted with a ring during the operation, mainly to increase the amount of water injected, raise the temperature, and increase the contact area with the organs. Some scholars in China use hot distilled water for postoperative irrigation

2. Simple hyperthermic perfusion chemotherapy

Heat the saline solution to 45-48℃, inject it quickly, and gently massage the abdomen to increase diffusion and reduce the high temperature area. The dual-channel abdominal method is to insert at least two temperature probes at the same time, and a channel can be opened on the opposite side to lead out saline to increase the abdominal temperature.

3. Circulatory hyperthermic perfusion chemotherapy

Without opening the abdomen, a water inlet and outlet tube are inserted, and then the patient is reheated outside the body. This method has high requirements on equipment. In addition, the water outlet is easily blocked by the intestine, so special treatment must be done on the water outlet to prevent the intestine from being sucked in. In order to solve the problem of water circulation, the abdominal cavity indwelling tube can be buried subcutaneously. This method can be retained for a long time and is easy to use. The specific method is to make a small incision in the abdominal wall and insert a special "Y"-shaped double-lumen tube into the vesicorectal pouch or upper abdomen along the inner abdominal wall. Then the incision of the greater omentum is tightly sutured, and the tube is passed along the subcutaneous tunnel for a while and then out of the abdominal wall. After the original incision is sutured and the exit skin is sutured around the tube, water can be injected and circulation can be started.

Hyperthermic intraperitoneal chemotherapy for the treatment of malignant ascites is suitable for:

1. Patients who were found to have cancer cells in the flushing fluid during pelvic malignancy surgery.

2. When surgery for abdominal and pelvic malignant tumors reveals sand-like metastasis to multiple organs, the mesentery, and the surface of the greater omentum, and no tumor has formed or only a tumor less than 0.5 cm has formed.

3. During surgery for malignant tumors of the digestive tract (especially gastric and intestinal tumors), it is found that the tumor has invaded the entire layer and the probability of implantation or lymph node metastasis is high.

4. During surgery for digestive tract tumors (especially stomach and intestinal tumors), when it is found that the tumor has invaded the entire layer and formed adhesions with the surrounding area and cannot be removed, intraperitoneal perfusion of thermal chemotherapy can be performed followed by surgery, which may achieve the goal of radical cure of the unresectable tumor.

5. If implantation metastasis occurs after surgery for abdominal or pelvic malignant tumors and the tumor is >2 cm, intraperitoneal thermochemotherapy can be performed after resection of the tumor visible to the naked eye.

6. Treatment of ascites caused by carcinomatous peritonitis.

If you want to use hot perfusion for chemotherapy, you must also make corresponding dietary improvements. It is crucial to eat more nutritious foods, and it is also very important to consume more high-calorie foods. This will allow you to resist and treat the disease more successfully.
